包皮环切联合系带切除术治疗早泄伴包皮系带过短患者的临床研究

摘　　要：目的探讨包皮环切联合系带切除术治疗早泄伴有包皮系带过短患者的临床效果.方法收集2019年3月至2020年5月所有来本院就诊的268例PE患者的临床资料,其中愿意接受包皮环切联合包皮系带切除术治疗且符合人组标准的患者45例.术前评估了阴道内射精潜伏时间(IELT)、勃起功能的基线水平、早泄症状调查评分(CHEES)、性生活满意度调查和阴茎龟头灵敏度测定.术后连续6个月通过电话或微信联系方式评估IELT、CHEES、性生活满意度、勃起功能和阴茎龟头灵敏度测定.结果术后2、4、6个月的IELT、性生活满意度较术前明显提高,而CHEES评分较术前降低,差异均有统计学意义(均P<0.001);手术前、后患者的阴茎勃起功能比较,差异无统计学意义(P=0.990).患者术后的阴茎龟头灵敏度明显降低,差异有统计学意义(P<0.001).结论包皮环切联合系带切除术是一种微创,易于执行,安全、经济的手术,能明显改善患者PE症状.一般情况下,只需使用局部麻醉即可进行,无明显手术相关并发症,患者的不适感极小,值得临床推广应用.Objective To investigate the clinical effect of circumcision combined with frenec-tomy in the treatment of premature ejaculation with short frenulum.Methods Clinical data of 268 PE patients who came to our hospital from March 2019 to May 2020 were collected,including 45 patients who were willing to receive circumcision combined with frenectomy and met the inclusion criteria.In-travaginal latent ejaculation time(IELT),baseline erectile function,premature ejaculation symptom survey score(CHEES),sexual satisfaction survey,and glans sensitivity were assessed preoperatively.IELT,CHEES,sexual satisfaction,erectile function and glans sensitivity were assessed by phone or wechat for 6 months after surgery.Results IELT and CHEES scores and sexual satisfaction at 2,4 and 6 months after operation were significantly higher than those before operation,with statistical significance(all P<0.001).There was no significant difference in penile erectile function between patients before and after surgery(P=0.990).The sensitivity of the glans of the penis was significantly decreased after surgery,with statistical significance(P<0.001).Conclusions Circumcision combined with frenectomy is a minimally invasive,easy-to-perform,safe and economical procedure that can significantly improve PE symptoms in patients.Under normal circumstances,only local anesthesia can be used,no obvious surgery-related complications,patients with minimal discomfort,worthy of clinical application.
