早期胃癌患者临床病理分型与中医舌象、证型的关系

摘　　要：目的:探讨早期胃癌病理分型与中医舌象及证型的关系,为早期胃癌患者的中医诊断提供理论依据。方法:选取山东省菏泽市中医医院中医内科收治的50例经病理组织学证实为胃癌的患者作为研究对象,收集患者的一般信息、舌象信息及中医四诊信息,并根据中医四诊资料进行中医辨证分型,依据胃黏膜病理切片确定临床病理分型。对不同病理分型和中医舌象及证型的关系进行分析。结果:早期胃癌病理分型以中、高分化腺癌、黏液腺癌居多,舌象以绛舌、黄腻苔居多,中医证型以脾胃湿热证居多;高分化腺癌的淡白舌、红舌高于其他病理分型,差异有统计学意义(P <0.05);高分化腺癌的黄腻苔、白腻苔的比例高于其他病理分型,差异有统计学意义(P <0.05);高分化腺癌的脾胃湿热证、脾胃亏虚证比例高于其他病理分型,差异有统计学意义(P <0.05)。结论:在早期胃癌不同病理分型中,舌象、中医证型均呈现特异性表现,早期胃癌病理分型与中医舌象、中医证型具有一定的相关性,可作为早期胃癌的诊断依据。Objective: To explore the relationship between pathological types of early gastric cancer and TCM tongue symptoms and syndrome types, and to provide theoretical basis for TCM diagnosis of patients with early gastric cancer. Methods: Fifty patients with gastric cancer confirmed by histopathology were selected as the research objects. The general information, tongue image information and information of TCM four diagnostics of patients were collected, and TCM syndrome differentiation was carried out according to the data of TCM four diagnostics, and clinicopathological types were determined based on gastric mucosal pathological sections;and the relationship between different pathological types and TCM tongue symptoms and syndrome types was analyzed. Results: The pathological types of early gastric cancer were mostly medium and well-differentiated adenocarcinoma, and mucinous adenocarcinoma. The tongue images were mostly red tongue and yellow greasy fur. The TCM syndrome types were mostly damp-heat syndrome in the spleen and stomach. The pale tongue and red tongue of well-differentiated adenocarcinoma were higher than other pathological types(P<0.05);the proportion of yellow greasy fur and white greasy fur in well-differentiated adenocarcinoma was higher than that in other pathological types(P<0.05);the proportion of spleen-stomach damp-heat syndrome and spleen-stomach deficiency syndrome in well-differentiated adenocarcinoma was higher than that in other pathological types, with statistical significance(P<0.05). Conclusion: In different pathological types of early gastric cancer, tongue symptoms and TCM syndrome types have specific manifestations. The pathological classification of early gastric cancer has a certain correlation with TCM tongue symptoms and TCM syndrome types, and can be used as the basis for the diagnosis of early gastric cancer.
