基于皮疹特点运用中西医理论探讨重症手足口病的预后及治疗

摘　　要：目的:以皮疹特点为切入点,依据中西医理论基础,探讨重症手足口病的预后与治疗。方法:回顾性总结2012-2020年河北医科大学附属河北省儿童医院收治确诊的54例重症手足口病患儿的临床资料,根据预后及皮疹特点分为存活组与死亡组,皮疹数个组与皮疹较多组,5 d内皮疹仍鲜红组与5 d内皮疹已灰暗组,比较组间资料。结果:54例患儿中,存活组37例,死亡组17例,两组相比,死亡组患儿皮疹部位较少,皮疹数量较少,皮疹颜色较灰暗(P <0.05);皮疹数个组23例,皮疹较多组31例,两组相比,皮疹数个组患儿发生神经系统、呼吸系统、消化系统、循环系统并发症的比率较高,存活率较低(P <0.05);5 d内皮疹仍鲜红组37例,5 d内皮疹已灰暗组17例,两组相比,5 d内皮疹已灰暗组患儿的存活率较低(P <0.05),两组患儿发生神经系统、呼吸系统、消化系统、循环系统并发症的比率的差异无统计学意义(P> 0.05)。结论:重症手足口病皮疹特点不同,其并发症及预后不同,可结合中西医理论采取综合治疗。Objective: Taking the characteristics of skin rash as a starting point, to explore the prognosis and treatment of severe hand-foot-mouth disease based on the theoretical basis of TCM and Western medicine. Methods: The clinical data of 54 children with severe hand-foot-mouth admitted to Hebei Children’s Hospital Affiliated to Hebei Medical University from 2012 to 2020 was retrospectively summarized. The patients were divided into the survival group and the death group, the several rashes group and more rashes group, the group showing bright red rash within 5 days and group showing gray rash within 5 days. Results: There were 37 cases in the survival group and 17 cases in the death group. The rash sites, number and color of the rash in the death group were less and darker than the survival group(P<0.05). There were 23 cases in the several rashes group, and 31 cases in the more rashes group. The incidence of complications of nervous system, respiratory system, digestive system and circulatory system in the several rashes group was higher and the survival rate was lower(P<0.05). There were 37 cases in the group showing bright red rash within 5 days, and 17 cases in the group showing gray rash within 5 days. The survival rate of children in the group showing gray rash within 5 days was lower(P<0.05), and there was no statistical difference in the rate of complications of the nervous system, respiratory system, digestive system and circulatory system between the two groups(P>0.05). Conclusions: The characteristics of severe hand-foot-mouth disease rash are different, and their complications and prognosis are different. Comprehensive treatment can be applied by combining TCM with Western medicine.
