从气机升降出入论心力衰竭的治疗

摘　　要：心力衰竭是多种心系疾病的终末阶段,其病位在心,但非独心病,涉及多脏,最终导致诸脏腑功能失调。心力衰竭的病因责之于久病、外感、情志、劳倦等,病机可用“虚”“瘀”“水”三者概括。在“心力衰竭”的辨证论治上,有心气不足,津血瘀阻者;有心阳虚衰,水津外泛者;有瘀血化水,气机郁滞者,归根到底都是气机升降出入失衡所致。文章从气机升降出入理论阐述五脏气机升降出入与心力衰竭之间的联系,认为气机升降出入失调是导致心力衰竭发生和发展的关键点,应用“气机升降出入”理论施治,为心力衰竭的中医治疗提供新的思路和方法。Heart failure is the terminal stage of a variety of cardiac system diseases.The disease is located in the heart,but not the heart alone,involves multiple organs and ultimately leads to dysfunction of various organs.The etiology of heart failure is chronic illness,exogenous pathogens,emotions disorder,fatigue and so on.The pathogenesis can be summarized as deficiency,stasis and Shui(水).In the diagnosis and treatment of heart failure based on TCM syndrome differentiation,there are patients with insufficient Qi(气)of heart and blood stasis obstructed;there are patients with Yang(阳)deficiency and failure of heart,and spilled Shui and body fluid;and there are patients with Shui converted from blood stasis,and Qi stagnation and so on.In the final analysis,these symptoms are all caused by abnormal ascending and descending of Qi movement.In this paper,the relationship between the ascending and descending of Qi movement in five internal organs and heart failure was expounded based on the theory of ascending and descending of Qi movement.It is believed that abnormal ascending and descending of Qi movement is the key point leading to the occurrence and development of heart failure,and treatment based on the theory of ascending and descending of Qi movement should be applied in clinical practice.It provides new ideas and methods for the treatment of chronic heart failure in TCM.
