加味逍遥汤联合雌孕激素周期疗法治疗月经不调(肝郁血虚脾弱型)患者的临床观察

摘　　要：目的:探讨加味逍遥汤联合雌孕激素周期疗法治疗月经不调(肝郁血虚脾弱型)患者的临床效果。方法:选取2019年12月-2020年12月经不调患者130例,采用随机数字表法将其分成观察组和对照组,各65例。对照组采用雌孕激素周期疗法,观察组联合加味逍遥汤治疗。对比两组治疗效果、中医症状评分、血清激素水平、月经周期天数、生活质量及不良反应发生率,随访3个月,比较复发率。结果:观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05);经治疗,两组两胁作痛、头晕目眩、精神疲乏、烦躁易怒症状评分均低于治疗前,观察组中医症状评分低于对照组,差异有统计学意义(P<0.05);治疗后,两组雌二醇水平高于治疗前,促黄体生成素、促卵泡生成素均低于治疗前,差异有统计学意义(P<0.05),观察组促黄体生成素、促卵泡生成素、雌二醇水平均优于对照组,差异有统计学意义(P<0.05);治疗后,观察组月经周期天数长于对照组,生活质量评分高于对照组,差异均有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05);随访3个月,观察组复发率显著少于对照组,差异有统计学意义(P<0.05)。结论:采用加味逍遥汤联合雌孕激素周期疗法治疗肝郁血虚脾弱型月经不调能明显改善患者的临床症状及血清激素水平,增加月经周期天数,提高生活质量,复发率低且未增加不良反应,值得临床运用。Objective:To investigate the clinical efficacy of the Jiawei Xiaoyao decoction(加味逍遥汤)plus estrogen and progesterone cycle therapy on patients with irregular menstruation(the liver stagnation,blood deficiency and spleen weakness type).Methods:A total of 130 patients with irregular menstruation from December 2019 to December 2020 were selected and divided into the observation group and the control group,with 65 cases in each.The control group was treated by estrogen and progesterone cycle therapy,while the observation group was treated with the Jiawei Xiaoyao decoction.The treatment effect,TCM symptom score,serum hormone level,menstrual cycle days,quality of life and incidence of adverse reactions were compared between the two groups,followed up for 3 months,and the recurrence rate was compared.Results:The total efficiency in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05);after treatment,both flank pain,dizziness,mental fatigue,irritability and irritability symptom scores in the two groups were all lower than those before treatment;the TCM symptom scores in the observation group were lower than those in the control group(P<0.05);after treatment,the E_(2) levels in the two groups were higher than those before treatment,and the LH and FSH in the observation group were lower than those before treatment(P<0.05).The levels of LH,FSH and E_(2) in the observation group were better than those in the control group(P<0.05).The menstrual cycle days in the observation group were longer than those in the control group,and the quality of life score was higher than that in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).After 3 months of followed-up,the recurrence rate in the observation group was significantly lower than that in the control group(P<0.05).Conclusion:The Jiawei Xiaoyao decoction plus estrogen and progesterone cycle therapy on irregular menstruation d
