补肾调肝合剂结合针刺在人工授精-胚胎移植反复移植失败患者的应用效果

摘　　要：目的:分析补肾调肝合剂结合针刺在肾虚肝郁型人工授精-胚胎移植(In Vitro Fertilization-Embryo Transfer,IVFET)反复移植失败患者治疗中的应用价值。方法:选取2018年1月-2020年6月就诊于广州市荔湾区中医医院的68例IVF-ET反复移植失败患者,随机分为两组,每组34例。西药组接受单纯西药拮抗剂促排治疗,联合组在西药组治疗基础上给予补肾调肝合剂结合针刺治疗。分析两组治疗前后中医证候积分变化情况,对比两组人绒毛膜促性腺激素注射日的雌二醇水平、孕激素水平、孕激素/雌二醇值。促性腺激素总量、获卵数、优质卵子数、受精率、优质胚胎率及妊娠率。结果:①两组治疗前中医证候评分的差异无统计学意义(P>0.05),而在治疗后,两组中医证候评分均显著下降,联合组显著低于西药组(P<0.01)。②联合组促性腺激素用量低于西药组(P<0.05),而两组人绒毛膜促性腺激素注射日雌二醇水平、孕激素水平及孕激素/雌二醇值比较,差异无统计学意义(P>0.05)。③两组间获卵数比较,差异无统计学意义(P>0.05),但联合组优质卵子数、受精率、优质胚胎率及妊娠率均高于西药组(P<0.01)。此外,两组治疗期间均无不良反应发生,两组间差异无统计学意义(P>0.05)。结论:补肾调肝合剂结合针刺可有效改善肾虚肝郁型IVF-ET反复移植失败患者的中医证候,减少促性腺激素用量,改善IVF-ET结局。Objective:To analyze the application value of the Bushen Tiaogan mixture(补肾调肝合剂)plus acupuncture on repeated IVF-ET transplantation failure patients with kidney deficiency and liver stagnation.Methods:A total of 68 cases from January 2018 to June 2020 were randomly divided into two groups,with 34 cases in each group.The Western medicine group received only Western medicine antagonists to promote excretion,and the combined group received the Bushen Tiaogan mixture plus acupuncture more.The changes of TCM syndrome scores in the two groups before and after treatment were analyzed.The E2,progesterone,progesterone/E2 ratio,total gonadotropin,number of eggs retrieved,number of high-quality eggs,fertilization rate,high-quality embryo rate and pregnancy rate in two groups were compared.Results:There was no significant difference in the scores of TCM syndromes between the two groups before treatment(P>0.05);but after the treatment,the TCM syndrome scores in the two groups were significantly decreased,and the combined group was significantly lower than the Western medicine group(P<0.01).The dosage of gonadotropin in the combined group was lower than that in the Western medicine group(P<0.05),and there was no significant difference in E_(2),progesterone and progesterone/E2 levels on the day of HCG injection between the two groups(P>0.05).There was no significant difference in the number of retrieved eggs between the two groups(P>0.05);but the number of high-quality eggs,fertilization rate,high-quality embryo rate and pregnancy rate in the combined group were higher than those in the Western medicine group(P<0.01).In addition,there was no adverse reaction in the two groups during the treatment period,and there was no significant difference between the two groups(P>0.05).Conclusion:The Bushen Tiaogan mixture plus acupuncture can effectively ameliorate the TCM syndromes in patients with the kidney deficiency and liver stagnation type repeated IVF-ET transplantation failure,reduce the dosage of gonadotropin,and impr
