老年高血压合并心力衰竭治疗中小剂量甲状腺素的应用效果

摘　　要：目的探讨老年高血压合并心力衰竭治疗中小剂量甲状腺素的应用效果。方法选2018年9月至2019年9月在本院接受治疗的老年高血压合并心力衰竭患者90例,按照随机抽签法分组,对照组45例采用常规方法治疗,观察组40例除常规治疗外,同时使用小剂量甲状腺素治疗。对比两组临床疗效及血清三碘甲状腺原氨酸(T_(3))、总甲状腺素(T_(4))、左心射血分数(LVEF)水平。结果经治疗,观察组临床疗效优于对照组(P<0.05)。治疗前,两组T_(3)、T_(4)、LVEF水平比较,差异无统计学意义(P>0.05);治疗后,观察组T_(3)、T_(4)、LVEF指标明显优于对照组(P<0.05)。治疗前,两组患者血脂、血压以及血清钙比较,差异无统计学意义(P>0.05);治疗后,观察组血脂、血压以及血清钙显著更优(P<0.05)。结论对老年高血压合并心力衰竭患者在使用常规治疗的基础上应用小剂量甲状腺素治疗,能够改善患者的血清T_(3)、T_(4)及LVEF水平,血脂及血压水平控制较佳,治疗效果良好。Objective To investigate the effect of treatment of moderate-dose thyroxine in elderly patients with hypertension and heart failure.Methods From September 2018 to September 2019,90 elderly patients with hypertension complicated with heart failure who received treatment in our hospital were selected according to random sampling method,45 cases in the control group were treated with method,and 40 cases in observation group were treated with low dose thyroxine in addition to routine treatment.The clinical efficacy and serum levels of triiodothyronine(T_(3)),total thyroid hormone(T_(4))and left ventricular ejection fraction(LVEF)were compared.Results After treatment,the clinical effect of observation group was better than that of observation group the control group(P<0.05).Before treatment,there was no significant difference in T_(3),T_(4),LVEF between the two groups(P>0.05).After treatment,the T_(3),T_(4),LVEF indexes in the observation group were significantly better than those in the control group(P<0.05).Before treatment,there was no significant difference in blood lipids,blood pressure and serum calcium between the two groups(P>0.05);after treatment,blood lipids,blood pressure and serum calcium in the observation group were significantly better(P<0.05).Conclusion The use of small-dose thyroxine treatment on the basis of routine treatment for elderly patients with hypertension and heart failure can improve the serum triiodine thyrogenine,total thyroxine and left cardiac ejection fraction level,better blood lipid and blood pressure level control,and the treatment effect is good.
