参麦清热滋阴方减轻头颈部肿瘤放射性口腔黏膜炎的作用  

Effect of Professor Shenmai Qingre Ziyin Recipe on Reducing Radiation Oral Mucositis of Head and Neck Tumors

摘　　要：目的观察林毅教授所创的参麦清热滋阴方对头颈部肿瘤放疗相关口腔黏膜炎的作用。方法回顾性收集在广州中医药大学第二附属医院放疗科2020年1月至2021年12月接受规范的放射治疗的Ⅱ~Ⅳa期头颈部肿瘤130例。根据放疗过程中有无服用中药参麦清热滋阴方分为观察组(68例)、对照组(62例)。两组患者从开始出现放射性口腔黏膜炎时均接受了西药含漱治疗:地塞米松液+维生素B12液+利多卡因+0.9%生理盐水组成的漱口液,每6 h漱口1次至放疗结束1周。观察组除以上西药含漱处理之外,从放疗开始每日服用中药参麦清热滋阴方,至放疗结束后2周。按WHO关于急性放射性口腔黏膜炎分级标准、中国版肿瘤患者生活质量量表(QOL),比较两组患者出现3级以上放射性的口腔黏膜炎的比例及出现3级放射性口腔黏膜炎的放疗次数,在放疗结束时放射性口腔黏液炎的分级、生活质量、口腔黏膜恢复所需时间。结果观察组在放疗过程中出现3级放射性口腔黏膜炎的例数较对照组少,出现时间较对照组延后,两组比较差异有统计学意义(P<0.05)。观察组、对照组在放疗结束时放射性口腔黏膜炎分级分布方面,观察组严重放射性口腔黏膜炎占比低于对照组(P<0.05)。在放疗结束时,观察组患者的生活质量优于对照组(P<0.05)。在放疗结束时,观察组放射性口腔黏膜恢复所需时间短于对照组(P<0.05)。结论参麦清热滋阴方可延缓较重的放射性口腔黏膜炎出现的时间,减轻放疗结束时放射性口腔黏膜炎,提高放疗结束时的生活质量,缩短放疗结束后放射性口腔黏膜炎恢复时间。Objective To observe the effect of professor LIN Yi’s shenmai qingre ziyin recipe created,a master of Chinese medicine,on the response of oral mucositis related to radiotherapy of head and neck tumors.Methods A total of 130 patients with stageⅡ-Ⅳa head and neck tumors who received standard radiotherapy in the Department of Radiotherapy of the Second Affiliated Hospital of Guangzhou University of Traditional Chinese Medicine from January 2020 to December 2021 were collected retrospectively.According to whether shenmai qingre ziyin recipe was taken during radiotherapy,it was divided into observation group(68 cases)and control group(62 cases).Both groups of patients received western medicine gargle treatment from the beginning of radiation oral mucositis:gargle composed of dexamethasone solution+vitamin Bsolution+lidocaine+0.9%normal saline,and gargle regularly q6h until the end of radiotherapy for 1 week.In addition to the above western medicine gargling treatment,the observation group took shenmai qingre ziyin formula every day from the beginning of radiotherapy to 2 weeks after the end of radiotherapy.According to the WHO grading standard for acute radiation oral mucositis and the Chinese version of quality of life(QOL)for tumor patients.The differences in the number of cases of oral mucositis with radiation above grade 3 and the number of radiotherapy in case of oral mucositis with radiation above grade 3 were compared between the two groups.The grade difference of radiation oral mucositis at the end of radiotherapy,the grading difference of radiation oral mucositis at the end of radiotherapy,the quality of life of patients at the end of radiotherapy and the time required for the recovery of oral mucositis after radiotherapy were compared between the two groups.Results The number of cases of grade 3 radiation oral mucositis in the observation group was less than that in the control group,and the occurrence time was delayed than that in the control group.There was significant difference between the two grou
