处方点评对促进和平里社区药物合理使用的效果与分析

摘　　要：目的对北京市和平里医院紧密型医连体和平里社区8个卫生服务站的用药情况进行合理性集中点评后的处方点评数据进行比较.分析社区用药的持续改进情况,促进基层合理用药.方法以《医院处方点评管理规范(试行)》实施细则、北京市医疗机构处方专项点评指南等为依据,由该院临床药学组药师参与点评2020年4月-2021年12月社区卫生服务站共计76565张处方,采用全部纸版处方人工审核,大幅提高了点评的精确度,对不合理处方进行对比分析.结果该院药师对社区卫生服务站的处方进行点评开始,处方的不合理率从自2020年4月的13.32%下降到2021年12月的3.96%,不合理率大幅下降.处方问题以用法用量不适宜、适应证不适宜为主,共占到了总处方的71.53%.结论通过实施处方的集中点评,使不合理率显著下降,促进了社区合理用药,提升了社区卫生服务站的管理及诊疗水平.Objective Hepingli hospital's pharmacists conducted prescription reviews of medication use in eight health service stations and compared prescription review data.Analyze the continuous improvement of community medication use and promote rational medication use at the grassroots level.Methods Based on the implementation rules of"Hospital Prescription Review Management Standards(for Trial Implementation)"and the guidelines for special review of prescriptions in Beijing medical institutions,pharmacists from our clinical pharmacy group participated in reviewing a total of 76,565 prescriptions in community health service stations from April 2020 to December 2021,using all paper prescriptions for manual review,which significantly improved the accuracy of the review and compared and analyzed unreasonable prescriptions.Results Since our pharmacist started to review the prescriptions of community health service stations,the unreasonable rate of prescriptions has decreased significantly from 13.32%since April 2020 to 3.96%in December 2021.Prescription problems were dominated by inappropriate dosage and inappropriate indications,which together accounted for 71.53% of the total prescriptions.Conclusion Through the implementation of the centralized review of prescriptions,the unreasonable rate has significantly decreased,promoting the rational use of medication in the community and improving the management and treatment level of community health service stations.
