血浆sCD36水平与冠心病合并非酒精性脂肪性肝病的相关性

摘　　要：目的研究血浆可溶性CD36(sCD36)与冠心病合并非酒精性脂肪性肝病(NAFLD)的相关性。方法比较35例冠心病合并NAFLD患者(A组)、35例单纯冠心病患者(B组)和35例健康志愿者(C组)血浆sCD36水平和生化、炎症相关指标。计算A组患者NAFLD纤维化评分(NFS)。分析A组上述各项指标与血浆sCD36水平的相关性,多重线性回归分析血浆sCD36水平的影响因素。结果A组和B组血浆sCD36、超敏C反应蛋白(hsCRP)、TNF-α和IL-6水平高于C组(P<0.05),且A组上述指标亦高于B组(P<0.05)。A组血浆sCD36水平与BMI、AST、ALT、hsCRP、TNF-α、IL-6和NFS呈正相关(r=0.800、0.865、0.797、0.957、0.909、0.907和0.527,P<0.05)。多重线性回归分析结果显示,hsCRP、TNF-α和IL-6是冠心病合并NAFLD患者血浆sCD36水平的影响因素(P<0.05)。结论冠心病合并NAFLD患者血浆sCD36水平升高,与肝纤维化有关,可能作为冠心病合并NAFLD的血浆标志物。sCD36可能通过炎症机制参与冠心病合并NAFLD的发生和发展。Objective To study the correlation between plasma soluble CD36(sCD36)and coronary heart disease(CHD)complicated with nonalcoholic fatty liver disease(NAFLD).MethodsPlasma sCD36 level and biochemical and inflammatory related indexes were compared among 35 patients with CHD complicated with NAFLD(group A),35 patients with simple CHD(group B)and 35 healthy volunteers(group C).NAFLD fibrosis score(NFS)in group A was calculated.The correlation between the above indexes and plasma sCD36 level was analyzed,and the influencing factors for plasma sCD36 level were analyzed by multiple linear regression analysis in group A.ResultsPlasma levels of sCD36,high-sensitivity C-reactive protein(hsCRP),TNF-αand IL-6 in groups of A and B were higher than those in group C(P<0.05),which in group A were higher than those in group B(P<0.05).Plasma sCD36 level in group A was positively correlated with BMI,AST,ALT,hsCRP,TNF-α,IL-6 and NFS(r=0.800,0.865,0.797,0.957,0.909,0.907 and 0.527,P<0.05).Multiple linear regression analysis showed that hsCRP,TNF-αand IL-6 were the influencing factors for plasma sCD36 level(P<0.05).Conclusion The increase of plasma sCD36 level in the patients with CHD complicated with NAFLD is related to liver fibrosis,which may be taken as a plasma marker.sCD36 may participate in the occurrence and development of CHD complicated with NAFLD through inflammatory mechanism.
