摘  要:目的探讨肺结核患者血清微小RNA-192(miR-192)和外周血单个核细胞(PBMC)中核苷酸结合寡聚化结构域蛋白2(NOD2)mRNA表达及其临床意义。方法实时荧光定量PCR检测肺结核患者(A组,120例)和健康体检者(C组,120例)血清miR-192和PBMC中NOD2 mRNA表达水平,分析A组两者在不同临床病理指标下表达情况。采用免疫比浊法、免疫化学发光法和ELISA法检测两组血清CRP、降钙素原(PCT)和IL-6水平。分析血清miR-192和PBMC中NOD2 mRNA表达与炎症因子的相关性,绘制ROC曲线分析血清miR-192和NOD2 mRNA表达对肺结核的诊断价值。结果A组血清miR-192表达降低,而PBMC中NOD2 mRNA表达升高(P<0.01)。A组血清miR-192和PBMC中NOD2 mRNA表达在不同年龄以及有无吸烟史、糖尿病史、HIV感染史和不同感染程度方面比较均有统计学差异(P<0.05)。A组血清CRP、PCT和IL-6水平较C组升高(P<0.01)。血清miR-192表达与血清CRP、PCT和IL-6水平呈负相关(P<0.05),而PBMC中NOD2 mRNA表达与血清CRP、PCT和IL-6水平呈正相关(P<0.05)。联合检测血清miR-192和PBMC中NOD2 mRNA表达对肺结核的诊断效能优于两者单独检测(P<0.01)。结论血清miR-192和PBMC中NOD2 mRNA表达对肺结核具有较高的诊断价值。Objective To investigate the expression and clinical significance of serum microRNA-192(miR-192)and nucleotide-binding oligomerization domain protein 2(NOD2)mRNA in peripheral blood mononuclear cells(PBMC)of the patients with pulmonary tuberculosis.MethodsExpression levels of serum miR-192 and NOD2 mRNA in PBMC of 120 patients with pulmonary tuberculosis(group A)and 120 healthy controls(group C)were detected by real-time fluorescence quantitative PCR,which under different clinicopathological indicators in group A were analyzed.Serum levels of CRP,procalcitonin(PCT)and IL-6 in both groups were detected by immunoturbidimetry,immunochemiluminescence and ELISA,respectively.The correlations of the expressions of serum miR-192 and NOD2 mRNA in PBMC with inflammatory factors were analyzed.ROC curve was drawn to analyze the diagnostic value of serum miR-192 and NOD2 mRNA in PBMC for pulmonary tuberculosis.Results The expression of serum miR-192 was decreased,while the expression of NOD2 mRNA in PBMC was increased in group A(P<0.01).The expressions of serum miR-192 and NOD2 mRNA in PBMC in group A were significantly different in age,smoking history,diabetes history,HIV infection history and infection degree(P<0.05).Serum levels of CRP,PCT and IL-6 in group A were higher than those in group C(P<0.01).Serum miR-192 expression was negatively,while NOD2 mRNA expression in PBMC was positively correlated with serum levels of CRP,PCT and IL-6(P<0.05).The combined detection of serum miR-192 and NOD2 mRNA in PBMC had better diagnostic performance for pulmonary tuberculosis than serum miR-192 or NOD2 mRNA in PBMC alone(P<0.01).Conclusion The expressions of serum miR-192 and NOD2 mRNA in PBMC have higher diagnostic value for pulmonary tuberculosis.
