摘  要:目的观察克林霉素磷酸酯注射液对大鼠心电图和血流动力学的急性影响。方法将32只雄性SD大鼠随机分为空白对照组和克林霉素低、中、高3个剂量(50,100,200 mg/kg)组。大鼠经股静脉插管后恒速给药,采用标准肢体II导联和侵入法(液压传导)分别监测120 min内克林霉素磷酸酯注射液对大鼠心电图和血流动力学的影响,比较给药前后心电图及血流动力学变化。结果克林霉素磷酸酯注射液对心电图的影响主要表现在中、高剂量组分别在给药后0~30 min和0~120 min P-R间期、Q-T间期和QRS间期延长,与给药前相比,差异有统计学意义(P<0.01或P<0.05)。对血流动力学的影响主要表现在中、高剂量组分别在给药后120 min和0~120 min心率明显下降,与给药前相比,差异有统计学意义(P<0.01或P<0.05);中、高剂量组左室压力最大上升速率(+dp/dt_(max))和左室收缩峰压(LVSP)分别在给药后60~120 min、0~120 min明显下降,与给药前相比,差异有统计学意义(P<0.01);中、高剂量组的心室收缩开始至室内压变化速率达最大值的时间(t-dp/dt_(max))分别在给药后30~120 min和60~120 min明显增加,与给药前相比,差异有统计学意义(P<0.05)。结论克林霉素磷酸酯注射液在临床用药条件下未对大鼠心脏的心电图和血流动力学产生明显影响,但中、高剂量克林霉素磷酸酯注射液可减慢房室传导,使心室复极化和心室除极化过程减慢,有明显的使心肌收缩功能下降的表现。建议该药在临床应用时监控心脏不良反应。Objective To observe the acute effect of Clindamycin Phosphate Injection on electrocardiogram and hemodynamics of rats.Methods 32 Sprague-Dawley(SD)male rats were randomly divided into control group and low,medium and high dose(50,100 and 200 mg/kg)groups.The drug was administered to the rats through the femoral vein canula at a constant rate.The standard limb II lead and invasive method(hydraulic conduction)were used to monitor the effects of Clindamycin Phosphate Injection on electrocardiogram and hemodynamics of rats in 120 minutes.The changes of electrocardiogram and hemodynamics before and after drug administration were observed.Results In medium and high dose groups,prolonged P-R interval,Q-T interval and QRS interval at 0-30 min and 0-120 min after drug administration were observed,respectively.Compared with the indexes observed before drug administration,the difference was statistically significant(P<0.01 or P<0.05).The influence of Clindamycin Phosphate Injection on hemodynamics was mainly manifested in the heart rhythms of animals in the middle and high dose groups at 120 min and 0-120 min after drug administration,respectively.The difference between the heart rhythms before and after drug administration was statistically significant(P<0.01 or P<0.05).In the middle and high dose groups,+dp/dt_(max) and LVSP decreased significantly at 60-120min and 0-120 min after the drug administration,respectively.The difference of the indexes before and after drug administration was statistically significant(P<0.01).In the middle and high dose groups,t-dp/dt_(max) increased at 30-120 min and 60-120 min,respectively,and the difference between the index detected before and after drug administration was statistically significant(P<0.05).Conclusion Under the condition of clinical medication,Clindamycin Phosphate Injection has no effect on the electrocardiogram and hemodynamics.However,medium and high doses of Clindamycin Phosphate Injection can slow down the atrioventricular conduction,slow down the process of ventricula
