替格瑞洛联合奥扎格雷钠治疗脑血栓疗效观察

摘　　要：目的探讨替格瑞洛联合奥扎格雷钠治疗脑血栓的临床疗效,以及对患者血小板参数水平的影响。方法选取医院2018年5月至2021年5月收治的脑血栓患者114例,按随机抽签法分为观察组和对照组,各57例。两组患者均予奥扎格雷钠治疗,观察组患者加用替格瑞洛,均治疗2周。结果观察组总有效率为91.23%,显著高于对照组的75.44%(P<0.05)。治疗后,观察组患者的血小板计数、血小板黏附率、平均血小板体积均显著低于对照组(P<0.05);观察组患者的全血黏度、血浆黏度、红细胞变形指数、纤维蛋白原水平均显著低于对照组(P<0.05);观察组患者的脑损伤标志物包括神经元特异性烯醇化酶、胶质细胞原纤维酸性蛋白、同型半胱氨酸水平均显著低于对照组(P<0.05)。观察组和对照组患者治疗期间药品不良反应发生率相当(14.04%比10.53%,P>0.05)。结论替格瑞洛联合奥扎格雷钠治疗脑血栓临床疗效良好,可降低患者的血小板参数和血液黏度,改善脑损伤状态,且治疗安全性较好。Objective To investigate the clinical efficacy of ticagrelor combined with sodium ozagrel in the treatment of cerebral thrombosis,and its effect on the platelet parameters level of patients.Methods A total of 114 patients with cerebral thrombosis admitted to the hospital from May 2018 to May 2021 were selected and divided into the observation group and the control group according to the random lottery method,with 57 cases in each group.Patients in both groups were treated with sodium ozagrel,on this basis,patients in the observation group were treated with ticagrelor.Both groups were treated for two weeks.Results The total effective rate in the observation group was 91.23%,which was significantly higher than 75.44%in the control group(P<0.05).After treatment,the platelet count(PLT),platelet adhesion rate and mean platelet volume(MPV)in the observation group were significantly lower than those in the control group(P<0.05).After treatment,the levels of whole blood viscosity,plasma viscosity,erythrocyte deformation index and fibrinogen in the observation group were significantly lower than those in the control group(P<0.05).After treatment,the levels of brain injury markers such as neuron-specific enolase(NSE),glial fibrillary acidic protein(GFAP)and homocysteine(HCY)in the observation group were significantly lower than those in the control group(P<0.05).During the treatment,the incidence of drug adverse reactions in the observation group was similar to that in the control group(14.04%vs.10.53%,P>0.05).Conclusion Ticagrelor combined with sodium ozagrel is effective and safe in the treatment of cerebral thrombosis,which can decrease the level of platelet parameters and blood viscosity,and improve the brain injury.
