异位妊娠应用经阴道超声、经腹部超声及联合诊断的临床价值分析

摘　　要：目的 探究经阴道超声、经腹部超声及联合诊断异位妊娠的临床价值。方法 100例疑似异位妊娠患者,入院后分别行经阴道超声、经腹部超声诊断,以病理诊断结果作为金标准,比较经阴道超声、经腹部超声单独及联合诊断的敏感性、特异性与准确率。结果 病理诊断结果显示,100例患者中81例阳性,19例阴性;经腹部超声、经阴道超声诊断结果中真阳性分别为62、70例,真阴性分别为10、13例。联合诊断的敏感性、特异性及准确率分别为97.53%、94.74%、97.00%,经腹部超声分别为76.54%、52.63%、72.00%,经经阴道超声分别为86.42%、68.42%、83.00%。联合诊断的敏感性、特异性及准确率均高于经腹部超声、经阴道超声诊断,差异具有统计学意义(P<0.05);经阴道超声诊断的敏感性、特异性及准确率均高于经腹部超声,但差异无统计学意义(P>0.05)。结论 经阴道超声与经腹部超声联合诊断异位妊娠的敏感性、特异性、准确率高,能够提升对异位妊娠的检出率,为临床治疗提供依据,可予以推广。Objective To investigate the clinical value of transvaginal ultrasound,transabdominal ultrasound and their combined diagnosis of ectopic pregnancy.Methods 100 patients with suspected ectopic pregnancy were diagnosed by transvaginal ultrasound and transabdominal ultrasound after admission,and their sensitivity,specificity and accuracy of transvaginal ultrasound and transabdominal ultrasound alone and in combination were compared using pathological findings as the gold standard.Results The pathological findings showed that among 100 patients,81 cases were positive and 19 cases were negative.The number of positive cases detected by transabdominal ultrasound and transvaginal ultrasound were 61 and 70 cases,respectively,and the number of negative cases were 10 and 13 cases,respectively.The sensitivity,specificity,and accuracy of combined diagnosis were 97.53%,94.74%,and 97.00%,and those of transabdominal ultrasound were 76.54%,52.63%,and 72.00%,and those of transvaginal ultrasound were 86.42%,68.42%,and 83.00%.The sensitivity,specificity and accuracy of combined diagnosis were higher than those of transabdominal ultrasound and transvaginal ultrasound alone,and the difference was statistically significant(P<0.05).The sensitivity,specificity and accuracy of transvaginal ultrasound were higher than those of transabdominal ultrasound,but the difference was not statistically significant(P>0.05).Conclusion The combination of transvaginal ultrasound and transabdominal ultrasound has high sensitivity,specificity and accuracy in the diagnosis of ectopic pregnancy,which can improve the detection rate of ectopic pregnancy and provide a basis for clinical treatment,and can be promoted.
