GnRH-a联合曼月乐治疗子宫腺肌病的临床疗效分析

摘　　要：目的 探讨促性腺激素释放激素激动剂(GnRH-a)联合曼月乐对子宫腺肌病患者疼痛程度及月经量的影响。方法 74例子宫腺肌病患者,依据随机数字表法分为观察组与对照组,各37例。对照组患者应用GnRH-a治疗,观察组患者联合应用GnRH-a与曼月乐治疗。对比两组治疗前后痛经程度、月经量情况、治疗效果及卵巢功能指标。结果 治疗后3个月、6个月,观察组痛经程度评分均低于对照组,差异均有统计学意义(P<0.05)。治疗后3个月、6个月,观察组月经量评分均低于对照组,差异均有统计学意义(P<0.05)。观察组治疗总有效率97.30%高于对照组的83.78%,差异有统计学意义(P<0.05)。治疗后,观察组促黄体生成素(LH)、雌二醇(E_(2))分别为(2.51±1.35)U/L、(72.63±8.62)pmol/L,低于对照组的(4.30±1.20)U/L、(101.53±10.15)pmol/L;观察组促卵泡刺激素(FSH)为(11.26±2.63)U/L,高于对照组的(7.75±1.13)U/L,差异均有统计学意义(P<0.05)。结论 对于子宫腺肌病患者联合应用GnRH-a与曼月乐的治疗效果满意,可有效改善痛经程度和月经量,并可促使卵巢功能改善。Objective To discuss the effect of gonadotropin-releasing hormone agonist(GnRH-A)combined with Mirena on pain degree and menstrual volume in patients with adenomyosis.Methods A total of 74 patients with adenomyosis were divided into observation group and control group according to random numerical table,with 37 cases in each group.Patients in the control group were treated with GnRH-a,and patients in the observation group were treated with GnRH-a combined with Mirena.The degree of dysmenorrhea,menstrual volume before and after treatment,therapeutic effect and ovarian function indexes were compared between the two groups.Results After 3 and 6 months of treatment,the dysmenorrhea scores of the observation group were lower than those of the control group,and the differences were statistically significant(P<0.05).After 3 and 6 months of treatment,the menstrual volume scores in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The total effective rate 97.30%of the observation group was higher than 83.78%of the control group,and the difference was statistically significant(P<0.05).After treatment,the luteinizing hormone(LH)and estradiol(E_(2))in the observation group were(2.51±1.35)U/L and(72.63±8.62)pmol/L,which were lower than(4.30±1.20)U/L and(101.53±10.15)pmol/L in the control group;the follicle-stimulating hormone(FSH)(11.26±2.63)U/L was higher than(7.75±1.13)U/L in the control group;all the differences were statistically significant(P<0.05).Conclusion The combined treatment of GnRH-a and Mirena has a satisfactory effect for patients with adenomyosis,which can effectively improve pain degree and menstrual volume,and promote the improvement of ovarian function.
