对早发性精神分裂症谱系障碍患儿临床特征及疗效影响因素的分析

摘　　要：目的:探究早发性精神分裂症谱系障碍(EOS)患儿的临床特征及疗效影响因素。方法:回顾性分析宜昌市优抚医院2019年1月至2020年12月收治的98例EOS患儿的临床资料,根据起病年龄的不同将患儿分为儿童阶段组(起病年龄≤13岁)与青少年阶段组(起病年龄>13岁)。比较两组患儿的临床特征,并分析其疗效的影响因素。结果:与青少年阶段组患儿相比,儿童阶段组患儿生长发育的正常率更低,其言语及运动障碍的发生率、其中发病原因为家庭问题、临床症状表现为孤独患儿的占比均更高,差异有统计学意义(P<0.05)。进行Logistic回归分析的结果显示,合并精神发育迟缓或阿斯伯格综合征(OR=0.451,P=0.045)、孤独(OR=0.398,P=0.017)是EOS疗效的影响因素。结论:儿童阶段起病的EOS患儿存在更多的发育问题,且其疗效通常较差。孤独、合并精神发育迟缓或阿斯伯格综合征是其疗效的影响因素。Objective:To explore the clinical characteristics and influencing factors of curative effect in children with earlyonset schizophrenia spectrum disorder(EOS).Methods:Clinical data of 98 EOS children admitted to Yichang Special Care Hospital from January 2019 to December 2020 were retrospectively analyzed,and the children were divided into childhood stage group(onset age≤13 years old)and adolescent stage group(onset age>13 years old)according to different onset age.The clinical characteristics of the two groups were compared and the influencing factors of efficacy were analyzed.Results:Compared with the adolescent group,the normal rate of growth and development of children in the childhood group was lower,and the incidence of speech and movement disorders,family problems and loneliness accounted for a higher proportion of children,the difference was statistically significant(P<0.05).Logistic regression analysis showed that the combination of mental retardation OR Asperger’s syndrome(OR=0.451,P=0.045)and loneliness(OR=0.398,P=0.017)were the influencing factors of EOS efficacy.Conclusions:Children with childhood ONSET EOS have more developmental problems and generally have poor outcomes.Loneliness,mental retardation and Asperger’s syndrom e were the influencing factors.
