Detection of Serum Hcy and CysC Levels in Patients with Chronic Obstructive Pulmonary Disease and Respiratory Failure and Its Clinical Significance

摘  要:为探讨慢性阻塞性肺疾病(COPD)合并呼吸衰竭患者血清同型半胱氨酸(Hcy)、胱抑素C(CysC)的检测水平及其临床意义,选取宁夏医科大学总医院呼吸内科2016年1月—2019年12月收治的164例COPD患者为研究对象,其中伴呼吸衰竭80例(A组),不伴呼吸衰竭84例(B组);选取同期入院健康体检者80例作为对照组,检测各组血清一氧化氮(NO)、Hcy、CysC与动脉血氧分压(PaO_(2))水平并进行比较。依据入院时血清Hcy、CysC将A组患者分为高Hcy组、正常Hcy组和高CysC组、正常CysC组,分别比较其PaO_(2)水平、机械通气时间、住院时间和预后情况。结果显示:A组、B组和对照组NO、Hcy、CysC与PaO_(2)水平差异均有统计学意义(P<0.05);两两比较经LSD-t检验显示A组Hcy、CysC显著高于B组和对照组(P<0.05),NO、PaO_(2)水平则显著低于B组和对照组(P<0.05);B组Hcy、CysC显著高于对照组(P<0.05),NO、PaO_(2)水平则显著低于对照组(P<0.05);正常Hcy组、正常CysC组PaO_(2)水平、机械通气时间、住院时间和预后均优于高Hcy组、高CysC组(P<0.05)。该结果提示COPD合并呼吸衰竭患者血清Hcy、CysC水平变化与疾病进展密切相关,血清高Hcy、CysC水平患者病情更严重,病死率明显升高,可作为临床评估病情进展和预后的客观指标。The purpose of this paper is to explore the detection levels and clinical significance of serum homocysteine(Hcy)and cystatin C(CysC)in patients with chronic obstructive pulmonary disease(COPD)and respiratory failure.The study selected 164 patients with COPD and respiratory failure who admitted to the Department of Respiratory Medicine,General Hospital of Ningxia Medical University from January 2016 to December 2019 as the study subjects,including 80 patients with respiratory failure(group A)and 84 patients without respiratory failure(B Group).80 patients who were admitted to the hospital during the same period of health examination were selected as the control group.Serum nitric oxide(NO),Hcy,CysC and arterial partial pressure of oxygen(PaO_(2))levels in each group were detected and compared.According to serum Hcy and CysC at the time of admission,patients in group A were divided into high Hcy group,normal Hcy group,high CysC group,and normal CysC group.Their PaO_(2) levels,mechanical ventilation time,hospital stay and prognosis were compared.The results showed that the levels of NO,Hcy,CysC and PaO_(2) in A,B and the control group were statistically significant(P<0.05).Pairwise comparisons showed that Hcy and CysC in group A were significantly higher than those in group B and control group by LSD-t test(P<0.05).The levels of NO and PaO_(2) were significantly lower than those of group B and the control group(P<0.05).The Hcy and CysC of group B were significantly higher than those of the control group(P<0.05),and the levels of NO and PaO_(2) were significantly lower than those of the control group(P<0.05).Normal Hcy group and normal CysC group PaO_(2) level,mechanical ventilation time,hospital stay and prognosis were better than high Hcy group and high CysC group(P<0.05).The results suggest that changes in serum Hcy and CysC levels in patients with COPD and respiratory failure are closely related to disease progression.Patients with high serum Hcy and CysC levels are more severely ill and have a significantly hig
