穴位自血疗法联合盐酸左西替利嗪分散片治疗慢性荨麻疹临床观察

摘　　要：目的:观察穴位自血疗法联合盐酸左西替利嗪分散片治疗慢性荨麻疹的临床疗效。方法:将慢性荨麻疹患者120例随机分为治疗组和对照组各60例。治疗组口服盐酸左西替利嗪分散片+自血穴位注射治疗,对照组口服盐酸左西替利嗪分散片+穴位注射等容积生理盐水治疗,2天1次,连续治疗1个月。观察两组患者治疗前后皮肤瘙痒情况、风团大小和数量、症状体征总积分变化情况,检测患者血清细胞因子白细胞介素2(interleukin-2,IL-2)、白细胞介素4(interleukin-4,IL-4)、白细胞介素9(interleukin-9,IL-9)和干扰素γ(interferon,IFN-γ)的含量,记录有效率、复发率及不良反应情况。结果:治疗组能改善症状体征积分,总有效率高于对照组(P<0.05),复发率低于对照组(P<0.01);且不良反应发生率与对照组比较差异无统计学意义(P>0.05);治疗后两组患者IL-2和IFN-γ水平低于治疗前(P<0.01),治疗组低于对照组(P<0.05);治疗后两组IL-4和IL-9高于治疗前(P<0.01),治疗组高于对照组(P<0.05)。结论:穴位自血疗法联合盐酸左西替利嗪分散片可改善慢性荨麻疹患者临床症状和体征,提高治疗效果,降低复发率,安全性好;其机制可能与调节Th1/Th2细胞因子比例,调控免疫偏移有关。Objective:To observe clinical effects of acupoint self-blood therapy jointed with levocetirizine hydrochloride tablets in treating chronic urticaria.Methods:A total of 120 patients were randomized into the treatment group and the control group,60 cases in each group.The treatment group took levocetirizine hydrochloride tablets orally and received acupoint self-blood injection therapy,and the control group accepted oral administration of levocetirizine hydrochloride tablets and acupoint injection of equal volume of normal saline,once every two days,treating for one month consecutively.To observe skin itching conditions,the quantity and size of wheal,as well as the changes of total scores of symptoms and body signs before and after treatment in both groups,to detect the contents of IL-2,IL-4,IFN-γand IL-9 in the serum of the patients,to record the effective rate,the relapse rate and adverse reaction.Results:The treatment group could improve the scores of symptoms and body signs,the effective rate of the treatment group was superior to that of the control group(P<0.05),the relapse rate was lower than that of the control group(P<0.01);and there was no difference in the incidence of adverse reaction when compared with the control group(P>0.05);after the treatment,the levels of IL-2 and IFN-γwere lower than these before the treatment(P<0.01),the treatment group was lower than the control group(P<0.05);after the treatment,the levels of IL-4 and IL-9 were higher than these before the treatment(P<0.01),the treatment group was higher than the control group(P<0.05).Conclusion:Acupoint self-blood therapy and lerocetirizine hydrochloride tablets could improve clinical symptoms and body signs of the patients,raise therapeutic effects,reduce the relapse rate with good safety;and its mechanism might be realized possibly through regulating Th1/Th2 cytokine ratio,and adjusting immune migration.
