清热排脓生肌汤口服加外敷治疗剖宫产术后腹壁切口感染临床观察

摘　　要：目的:观察清热排脓生肌汤口服加外敷治疗剖宫产术后腹壁切口感染的临床效果。方法:将60例剖宫产术后腹壁切口感染患者随机分为治疗组和对照组各30例,治疗组口服中药清热排脓生肌汤,每日1剂,早晚分服,并将药渣装入消毒好的袋子于切口处外敷;对照组予广谱抗生素抗感染及清创缝合。观察两组患者切口感染病原菌分布及构成比;采用视觉模拟评分量表(visual analogue scale,VAS)比较两组患者疼痛情况;观察两组患者感染创面愈合时间、平均住院时间及患者满意度。结果:切口分泌物和脓液标本中分离鉴定出病原菌113株,其中革兰阴性菌68株,革兰阳性菌45株;VAS评分治疗组低于对照组(P<0.05);感染创面愈合时间和平均住院时间治疗组短于对照组(P<0.05);治疗组患者满意度高于对照组(P<0.05)。结论:清热排脓生肌汤口服加外敷能降低剖宫产术后腹壁切口感染,减轻疼痛,缩短创面愈合时间和平均住院时间,提高患者满意度,效果优于广谱抗生素抗感染及清创缝合。Objective:To observe therapeutic effects of oral administration and external application of Qingre Painong Shengji Tang in the treatment of abdominal incision infection after cesarean section.Methods:Sixty patients were randomized into the treatment group and the control group,30 cases in each group,the treatment group took Qingre Painong Shengji Tang orally,one dose each day,taking in the morning and evening,after putting the herb residues into the sterilized bags,it was externally applied;the control group received widespectrum antibiotics to resist infection,debridement and suture.To observe distribution and composition ratio of pathogenic bacteria in wound infection between both groups;VAS was used to compare the pain degrees between both groups;to observe healing time of infected wound,average hospital stay and the patients'satisfactions between both groups.Results:All 113 strains of pathogen were isolated and identified from the samples of incisional secretions and pus,among them,there were 68 strains of gram-negative bacteria and 45 ones of grampositive bacteria;VAS scores of the treatment group were lower than these of the control group(P<0.05);the treatment groupwas shorter than the control group in healing time of infectedwound and average hospital stay(P<0.05);the treatment group was higher than the control group in the patients'satisfactions(P<0.05).Conclusion:Oral administration and external application of Qingre Painong Shengji Tang could lower the incidence of abdominal incision infection after cesarean section,relieve pain,shorten healing time of infected wound and average hospital stay,and raise patients'satisfactions,and its effects are superior to these of wide-spectrum antibiotics resisting infection,debridement and suture.
