摘  要:目的探讨激光人工皱缩在单囊胚玻璃化冻融周期中的应用价值。方法回顾性分析296例行单囊胚玻璃化冻融移植的患者的临床资料。根据囊胚玻璃化冷冻前是否进行激光人工皱缩,将患者分为人工皱缩组(n=158)和非人工皱缩组(n=138),比较两组囊胚复苏率及囊胚移植后的妊娠结局。结果两组囊胚复苏率差异无统计学意义(P>0.05)。人工皱缩组的临床妊娠率、胚胎种植率和活产率均高于非人工皱缩组(均P<0.05),两组的生化妊娠率、早期流产率和单卵双胎率差异均无统计学意义(均P>0.05)。结论在囊胚玻璃化冷冻前进行激光人工皱缩,可改善临床妊娠率和活产率,且不增加多胎妊娠率和早期流产率,在单囊胚玻璃化冻融周期中具有临床推广的价值。Objective To explore the application value of laser artificial shrinkage in the vitrified frozen-thawed cycle of single blastocyst.Methods The clinical data of 296 patients undergoing the vitrified frozen-thawed single blastocyst transfer were retrospectively analyzed.The patients were assigned to artificial shrinkage group(n=158)or non-artificial shrinkage group(n=138)according to whether laser artificial shrinkage was conducted before vitrified freezing of the blastocyst.The blastocyst recovery rate and the pregnancy outcome after blastocyst transfer were compared between the two groups.Results No statistically significant difference was found in the blastocyst recovery rate between the two groups(P>0.05).The artificial shrinkage group exhibited higher rates of clinical pregnancy,embryo implantation,and live birth as compared with the non-artificial shrinkage group(all P<0.05),and the rates of biochemical pregnancy,early abortion,and monozygotic twin depicted no statistically significant difference between the two groups(all P>0.05).Conclusion Conducting laser artificial shrinkage of blastocyst before vitrified freezing can improve the clinical pregnancy rate and live birth rate,and does not increase the multiple pregnancy rate and early abortion rate,exerting clinical promotion value in the vitrified frozen-thawed cycle of single blastocyst.
