国内应用DRGs评价医疗服务绩效的文献知识图谱分析

摘　　要：目的了解我国基于疾病诊断相关分组(DRGs)开展医院医疗绩效评价的研究进展情况,总结研究热点及存在的问题。方法在中国知网数据库检索应用DRGs评价医疗服务绩效的文献。通过中国知网数据库的可视化分析功能分析文献量随年份的变化情况,并运用CiteSpace 5.7.R5W软件共现作者、机构和关键词知识图谱。结果随着年份的增长,国内应用DRGs评价医疗服务绩效的文献逐年增多,以2019年产出文献量最多。在应用DRGs开展医院绩效评价的相关研究上,我国学者已形成了两个比较大的聚类,较有影响力的文献主要来源于北京市的相关单位和部门。基于DRGs的医院绩效评价的相关研究热点已经从早期的运用基本条件的研究,逐步发展到基于DRGs评价三级医院医疗服务绩效、基于DRGs结合多种手段进行评价。结论随着相关政策的出台,应用DRGs评价医疗服务绩效已经成为并将持续成为研究热点。但在我国不同地区该方面的研究发展程度不一致,不同机构、学者还应该加强该研究问题的学术交流,分享前沿学术成果,促进该领域研究和实践的发展。Objective To understand the research progress of hospitals'medical performance evaluation based on diagnosis related groups(DRGs)in China,and to summarize the research hotspots and existing problems.Methods The literature concerning DRGs applied to evaluating medical service performance was retrieved from the China National Knowledge Infrastructure database.The visual analysis function of the China National Knowledge Infrastructure database was used to analyze the changes of literature amount over years,and CiteSpace 5.7.R5W software was employed to co-produce knowledge maps of authors,institutions,and keywords.Results In China,literature concerning DRGs applied to evaluating medical service performance increased over years,and the literature produced in 2019 occupied the first position.In the related researches on the application of DRGs to carrying out hospital performance evaluation,Chinese scholars contributed to two relatively large clusters,and the more influential literature mainly came from the relevant institutions and departments of Beijing.The relevant research hotspots of hospital performance evaluation based on DRGs were gradually developed from the early researches using basic criteria to the DRGs-based evaluation of medical service performance in tertiary hospitals,and the evaluation based on DRGs combing multiple methods.Conclusion With the introduction of related policies,the application of DRGs to evaluating medical service performance has become and will be continue to become research hotspots.But the research development in this field is not consistent in different regions of China.Different institutions and scholars should strengthen the academic communications of this research problem,share cutting-edge academic achievements,and promote the development of researches and practices in this field.
