IgA肾病患者尿蛋白定量水平与临床病理特征的相关性分析

摘　　要：目的:分析IgA肾病(IgAN)患者尿蛋白定量水平与临床病理特征的相关性.方法:采取回顾性研究的方法,收集2018年1月~2022年2月本院收治的170例IgAN患者的临床资料,根据24h尿蛋白定量水平,将其分为轻度组(24h尿蛋白定量<1g,37例)、中度组(1g≤24h尿蛋白定量≤3.5g,113例)、重度组(24h尿蛋白定量>3.5g,20例),对比三组临床资料、临床指标水平及IgAN牛津病理分型结果.结果:轻、中、重度组患者的血肌酐(Scr)、血尿素氮(BUN)、尿酸(UA)水平差异显著,但组间性别、年龄、体质指数(BMI)、高血压、总胆固醇、低密度脂蛋白(LDL)及血IgA水平比较无显著差异.Pearson相关性分析结果显示,IgAN患者24h尿蛋白定量水平与其Scr、BUN、UA水平均呈正相关关系,相关系数有统计学意义.轻、中、重度组患者的IgAN牛津病理分型分布情况差异显著;两两比较结果显示,重度组患者E0、S0患者比例显著低于轻、中度组,M0、T0及C0患者比例均显著低于轻度组.结论:IgAN患者尿蛋白定量水平与临床病理特征存在一定的相关关系,随着尿蛋白定量水平的增加,其肾功能也会随之恶化,且病理改变也随之加重.Objective To analyze the correlation between urine protein quantitation and clinicopathological characteristics in patients with IgA nephropathy(IgAN).Methods A retrospective study was performed on the clinical data of 170 patients with IgAN admitted to the hospital between January 2018 and February 2022.According to 24h urine protein quantitation,they were divided into mild group(<1 g,37 cases),moderate group(1 g-3.5 g,113 cases)and severe group(>3.5 g,20 cases).The clinical data,levels of clinical indexes and Oxford pathological classifications of IgAN were compared among the three groups.Results After comparison,there were significant differences in levels of serum creatinine(Scr),blood urea nitrogen(BUN)and uric acid(UA)among the three groups,but there was no significant difference in gender,age,body mass index(BMI),hypertension,total cholesterol,low density lipoprotein(LDL)or IgA.The results of Pearson correlation analysis showed that level of 24h urinary protein quantitation was positively correlated with levels of Scr,BUN and UA.After comparison,there were significant differ-ences in Oxford pathological classifications of IgAN among the three groups.The results of pairwise comparison showed that proportions of E0 and S0 cases in severe group were significantly lower than those in mild and moderate groups,and proportions of M0,T0 and C0 cases were significantly lower than those in mild group.Conclusion There is certain correlation between level of 24h urinary protein quantitation and clinicopathological characteristics in IgAN patients.With the increase of urinary protein quantitation,renal function is deteriorated and pathological changes are aggravated.
