摘  要:目的:分析慢性阻塞性肺疾病急性加重期(AECOPD)合并肺动脉高压患者动脉血气分析指标、BNP及PCT、D-D变化及临床意义.方法:纳入本院于2017年4月~2019年4月临床确诊的195例AECOPD患者,予以彩色多普勒心动图检查肺动脉收缩压情况,将合并肺动脉高压的患者设为研究组(n=121),未合并肺动脉高压的患者设为对照组(n=74),比较实验室检查各指标(PaO_(2)、SaO_(2)、PaCO_(2)、PCT、BNP、D-D)情况,分析各实验室指标的表达差异及临床意义.结果:与对照组比较,研究组患者SaO_(2)、PaO_(2)水平明显降低,且PaCO_(2)、PCT、BNP、D-D水平升高;AECOPD合并肺动脉收缩压患者SaO_(2)、PaO_(2)水平随肺动脉收缩压升高而降低,而PaCO_(2)、PCT、BNP、D-D水平随肺动脉收缩压升高而上升;经Spearman相关进行分析,AECOPD合并肺动脉高压患者PaCO_(2)、PCT、BNP、D-D水平与肺动脉收缩压严重程度呈正相关性,而PaO_(2)、SaO_(2)水平与肺动脉收缩压严重程度呈负相关.结论:动脉血气分析指标、BNP及PCT、D-D水平变化与AECOPD合并肺动脉高压患者具有相关性,可根据其指标变化情况对于患者病情变化做有效评估.Objective To analyze the changes in analysis indexes of arterial blood gas,BNP,PCT and D-D clinical sig-nificance in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)and pulmonary hypertension.Methods 195 AECOPD patients who were clinically diagnosed in our hospital from April 2017 to April 2019 were selected.The color Doppler echocardiography was used to check the systolic pressure of pulmonary artery.The patients with pulmonary arterial hypertension were set as the research group(n=121),and the patients without pulmonary arterial hypertension were set as the control group(n=74).The indicators of laboratory tests(PaO_(2),SaO_(2),PaCO_(2),PCT,BNP,D-D)were compared,and the differences in the expression of laboratory indicators and their clinical significance were analyzed.Results Compared with those in the control group,the levels of SaO_(2) and PaO_(2) in the study group significantly reduced,and the levels of PaCO_(2),PCT,BNP,and DD increased.In patients with AECOPD and pulmonary systolic pressure,the levels of SaO_(2) and PaO_(2) decreased with the increasing of pulmonary systolic pressure,while the levels of PaCO_(2),PCT,BNP,and DD increased with the increasing of pul-monary systolic pressure.According to Spearman correlation analysis,PaCO_(2),PCT,BNP,D-D levels of AECOPD patients with pulmonary hypertension were positively correlated with the severity of pulmonary systolic pressure,while PaO_(2),SaO_(2) levels were negatively correlated with the severity of pulmonary systolic pressure.Conclusion The changes in analysis indexes of arterial blood gas,BNP and PCT,D-D are correlated with AECOPD patients with pulmonary hypertension.According to the changes in its indicators,the patient’s condition can be effectively evaluated.
