低温等离子射频消融术在咽喉部手术中的应用

摘　　要：目的:探讨低温等离子射频消融术在咽喉部手术中的应用。方法:选取2016年12月—2019年12月洛阳市第三人民医院200例行咽喉部手术治疗患者作为研究对象,按手术方案不同分为研究组(n=100)和对照组(n=100),对照组予以喉钳钳取术治疗,研究组予以低温等离子射频消融术治疗。对比两组患者优良率、围术期情况、术后3 d、6 d视觉模拟评分法(VAS)评分、嗓音功能、并发症发生率和复发情况。结果:研究组优良率较对照组高,差异有统计学意义(χ^(2)=5.207,P<0.05);研究组术中出血量少于对照组,手术时间和创口愈合时间短于对照组,差异有统计学意义(t=18.449、27.272、11.780,P<0.05);研究组患者术后3 d、6 d,咽部、喉部VAS评分较对照组低,差异有统计学意义(t=21.510、26.910、36.878、38.446,P<0.05);术后1个月,研究组嗓音功能shimmer、Jitter指标较对照组低(t=40.897、3.664,P<0.05);研究组并发症发生率与对照组对比,差异无统计学意义(χ^(2)=0.421,P>0.05);研究组复发率与对照组相比,差异无统计学意义(χ^(2)=2.696,P>0.05)。结论:采用低温等离子射频消融术在咽喉部手术中临床疗效好,具备出血量少、手术时间短、创口愈合时间短的特点,可减轻患者术后疼痛,改善嗓音功能,具备手术安全性,且复发情况少。Objective:To investigate the application of low temperature plasma radiofrequency ablation in pharyngeal and laryn⁃geal surgery.Methods:From December 2016 to December 2019,200 patients who underwent pharyngeal surgery in the hospital were selected and divided into study group and control group according to different surgical plans,with 100 cases in each group.The control group was treated with laryngeal forceps and the study group was treated with low-temperature plasma radiofrequency ablation.The excellent and good rate,perioperative conditions,visual analogue scale(VAS)scores at 3 and 6 d after operation,voice function,complication rate and recurrence were compared between the two groups.Results:The excellent and good rate in the study group was higher than that in the control group,and the difference was statistically significant(χ^(2)=5.207,P<0.05).The intraoperative blood loss in the study group was less than that in the control group,and the operation time and wound healing time were shorter than those in the control group,and the difference was statistically significant(t=18.449,27.272,11.780,P<0.05).The VAS scores of the pharynx and larynx in the study group were lower than those in the control group at 3 d and 6 d after opera⁃tion,and the difference was statistically significant(t=21.510,26.910,36.878,38.446,P<0.05).One month after operation,the shimmer and Jitter indexes of voice function in the study group were lower than those in the control group,the difference was statis⁃tically significant(t=40.897,3.664,P<0.05).The incidence of complications in the study group was compared with that in the con⁃trol group,the difference was not statistically significant(χ^(2)=0.421,P>0.05).The recurrence rate in the study group was compared with in the control group,the difference was not statistically significant(χ^(2)=2.696,P>0.05).Conclusion:Low-temperature plasma radiofrequency ablation has a good clinical effect in throat surgery.It has the characteristics of less blood loss,shorter operation time,a
