呼吸道集束化护理对肺癌切除术患者并发症及康复状况的影响

摘　　要：目的:分析呼吸道集束化护理对肺癌切除术患者并发症发生情况及康复状况的影响。方法:选取2018年3月—2020年3月广东省农垦中心医院收治的100例需要进行肺癌切除术治疗的肺原位癌患者作为研究对象,通过双盲选法将其分为对照组和试验组,每组各50例。对照组采用常规护理模式进行护理,试验组采用呼吸道集束化护理模式进行护理。比较两组患者护理前后的呼吸运动功能、并发症发生率、康复状况以及护理满意度情况。结果:两组患者术前6 min步行试验(6 MWT)结果及Borg呼吸困难评分量表(Borg)评分比较,差异无统计学意义(t=0.122、0.270,P>0.05);术后7 d,试验组6 MWT结果显著高于对照组,Borg评分显著低于对照组,差异有统计学意义(t=7.009、3.800,P<0.05)。两组患者并发症发生率比较,试验组显著低于对照组,差异有统计学意义(χ^(2)=6.775,P<0.05)。两组患者胸管留置时间、术后氧疗时间、术后住院时间比较,试验组显著低于对照组,差异有统计学意义(t=3.451、9.694、5.571,P<0.05)。两组患者护理满意度比较,试验组显著高于对照组,差异有统计学意义(χ^(2)=5.316,P<0.05)。结论:在肺癌切除术患者的护理中,呼吸道集束化护理不仅可以有效降低并发症发生率,还可以进一步加快患者康复进程,改善呼吸运动功能,提高护理满意度。Objective:To analyze the impact of clustered respiratory care on the occurrence of complications and rehabilitation of patients undergoing lung cancer resection.Methods:From March 2018 to March 2020,100 patients with lung cancer in situ who needed to undergo lung cancer resection were selected as the research objects,and they were divided into control group and experimental group by double-blind selection,with 50 cases in each group.The control group was treated with routine nursing mode,while the experimental group was treated with respiratory bundle nursing mode.The respiratory movement function,complica⁃tion rate,rehabilitation status and nursing satisfaction before and after nursing were compared between the two groups.Results:There was no statistically significant difference in the preoperative 6-minute walk test(6 MWT)results and Borg dyspnea scale(Borg)score between the two groups(t=0.122,0.270,P>0.05).7 d after operation,the 6 MWT results in the experimental group were significantly higher than those in the control group,and the Borg score was significantly lower than that in the control group,with statistically significant differences(t=7.009,3.800,P<0.05).The incidence of complications in the experimental group was significantly lower than that in the control group,and the difference was statistically significant(χ^(2)=6.775,P<0.05).The chest tube indwelling time,postoperative oxygen therapy time,and postoperative hospital stay in the experimental group were significant⁃ly lower than those in the control group,and the differences were statistically significant(t=3.451,9.694,5.571,P<0.05).The nursing satisfaction of the experimental group was significantly higher than that of the control group,and the difference was statisti⁃cally significant(χ^(2)=5.316,P<0.05).Conclusion:In the nursing of patients with lung cancer resection,respiratory cluster nursing can not only effectively reduce the incidence of complications,but also further accelerate the recovery process of patients,improve their respira
