Nd∶YAG激光对复发性口腔溃疡的临床效果和对咀嚼功能的影响研究

摘　　要：目的 探讨Nd∶YAG激光治疗复发性口腔溃疡的临床效果和对患者咀嚼功能的影响。方法 选择2019年6月至2021年6月,复发性口腔溃疡患者100例,随机数字表法分为对照组和观察组,每组患者各50例。两组均给予口服维生素B、维生素C、葡萄糖酸锌等方法干预后,对照组再采用西咪替丁治疗,每日4次,连续用药7 d为1个疗程;观察组再采用Nd∶YAG激光治疗,治疗光剂量为单脉冲能量60~100 mJ、频率20 Hz,总能量1.2~2.0 J。治疗后7 d评估两组患者的临床效果,比较两组患者咀嚼功能和炎性因子IL-1、IL-6和TNF-α水平的变化。治疗后1、3和6个月,记录不良反应发生率和复发率。结果 治疗后7 d两组患者咀嚼功能与治疗前比较,均得到明显改善;治疗后7 d,观察组咀嚼效能和咬合力均高于对照组(P<0.05);两组炎性因子IL-1、IL-6和TNF-α水平均低于治疗前(P<0.05);观察组炎性因子IL-1、IL-6、TNF-α水平低于对照组(P<0.05)。治疗期间,两组患者发生恶心、呕吐、黏膜水肿及黏膜坏死等不良反应发生率比较,差异无统计意义(P>0.05)。随访6个月,观察组复发率低于对照组(P<0.05)。结论 Nd∶YAG激光治疗能改善复发性口腔溃疡患者咀嚼功能,可降低炎性因子水平,安全性较高,能获得良好远期预后,值得推广应用。Objective To investigate the clinical effect of Nd:YAG laser on recurrent oral ulcers and its influence on chewing functions.Methods Totally 100 patients of recurrent oral ulcers treated over the period from Jun.2019 to Jun.2021 were selected and divided randomly into two groups,50 patients in each group.Conventional interventions were adopted for both groups.The patients in the control group were then treated with cimetidine,and those in the observation group with Nd:YAG laser.After 7 days’ treatment,the effect was evaluated,and a 3-month follow-up was completed to compare the chewing functions,inflammatory factors,safety and recurrence rate.Results Mastication functions were significantly improved after 7 days’ intervention in both groups,but the mastication efficiency and bite force of the observation group were higher than those of the control group(P<0.05).For both groups,the levels of inflammatory factors dropped upon 7 days’ treatment(P<0.05),and the levels of such inflammatory factors as IL-1,IL-6 and TNF-αin the observation group were lower than those in the control group(P<0.05).The incidence of nausea and vomiting,mucosal edema and mucosal necrosis in both groups during the treatment was not statistically significant(P>0.05).The 6-month follow-up recurrence rate of the observation group was lower than that of the control group(P<0.05).Conclusions Being highly safe,Nd:YAG laser treatment will improve the masticatory functions of patients with recurrent oral ulcers and reduce the levels of inflammatory factors.It will also obtain a good long-term prognosis and is worthy of popularization and application.
