摘  要:目的探讨创伤失血性休克中血清高迁移率蛋白B1(HMGB1)与肿瘤坏死因子-α(TNF-α)、白细胞介素(IL)-1β及IL-6的相关性。方法选取2020年1月至2021年6月期间聊城市第二人民医院诊治的54例创伤失血性休克患者及54例正常健康人为研究对象。Sprague-Dawley(SD)大鼠购自山东大学实验动物中心。构建创伤失血性休克SD大鼠模型并分为轻度休克组、中度休克组及重度休克组, 选择未行任何处理的SD大鼠为对照(对照组)。采用酶联免疫吸附法(ELISA)检测各组研究对象及SD大鼠血清HMGB1、TNF-α、IL-1β及IL-6水平并分析其相关性。两组间比较用独立样本t检验;多组间比较用单因素方差分析, 进一步两组间比较用LSD-t检验。相关性分析采用Pearson相关性分析。结果创伤失血性休克患者中血清HMGB1、TNF-α、IL-1β及IL-6水平均显著高于正常健康人, HMGB1为(85.38±23.92) ng/ml比(1.56±0.38) ng/ml, TNF-α为(147.28±32.38) pg/ml比(10.81±2.28) pg/ml, IL-1β为(170.28±49.28) pg/ml比(8.19±2.17) pg/ml, IL-6为(168.92±45.29) pg/ml比(105.98±27.56) pg/ml, 差异有统计学意义(t=25.751、30.895、24.147、8.723, 均P<0.05)。创伤失血性休克患者血清HMGB1与TNF-α、IL-1β、IL-6水平均呈正相关(相关系数r=0.797、0.611、0.731, P<0.05)。轻度休克组、中度休克组及重度休克组SD大鼠血清HMGB1、TNF-α、IL-1β及IL-6均显著高于对照组SD大鼠, HMGB1分别为(99.53±7.65)、(165.26±9.56)、(225.25±26.32) ng/ml比(35.01±4.84) ng/ml, TNF-α分别为(44.32±3.01)、(57.15±4.20)、(68.25±4.78) pg/ml比(24.11±2.02) pg/ml, IL-1β分别为(1.85±0.15)、(2.23±0.14)、(2.85±0.23) pg/ml比(1.02±0.11) pg/ml, IL-6分别为(88.56±8.14)、(154.15±19.58)、(201.23±15.50) pg/ml比(50.23±4.65) pg/ml, 差异有统计学意义(F=467.902、401.744、328.216、381.541, 均P<0.05)。创伤失血性休克SD大鼠血清HMGB1与TNF-α、IL-1β、IL-6相关性:创伤失血性休克SD大鼠血清HMGB1与TNF-α�Objective To study correlation between the serum levels of high mobility group box-1(HMGB1)and pro-inflammatory factors tumor necrosis factor-α(TNF-α),interleukin(IL)-1βand IL-6 in traumatic hemorrhagic shock patients and model rats.Methods A total of 54 patients with traumatic hemorrhagic shock and 54 people undergoing normal physical examination between January 2020 to June 2021 were selected.The SD rats were purchased from the Laboratory Animal Center of Shandong University.The SD rat model of traumatic hemorrhagic shock was constructed and divided into mild shock group,moderate shock group and severe shock group.The SD rats without any treatment were selected as control(control group).Enzyme-linked immunosorbent assay(ELISA)was used to detect the serum levels of HMGB1,TNF-α,IL-1βand IL-6 in each group of research subjects and SD rats,and their correlations were analyzed.The independent samples t test was used for comparison between two groups,the one-way analysis of variance was used for comparison between multiple groups,and the LSD-t test was used for further comparison between two groups.Correlation analysis was performed using Pearson correlation analysis.Results The serum levels of HMGB1,TNF-α,IL-1βand IL-6 in patients with traumatic hemorrhagic shock were significantly higher than those in normal healthy people[HMGB1:(85.38±23.92)vs.(1.56±0.38)ng/ml;TNF-α:(147.28±32.38)vs.(10.81±2.28)pg/ml;IL-1β:(170.28±49.28)vs.(8.19±2.17)pg/ml;IL-6:(168.92±45.29)vs.(105.98±27.56)pg/ml].The difference was statistically significant(t=25.751,30.895,24.147,8.723,all P<0.05).Serum HMGB1 was positively correlated with the levels of TNF-α,IL-1βand IL-6 in patients with traumatic hemorrhagic shock(correlation coefficients r=0.797,0.611 and 0.731,P<0.05).Serum HMGB1,TNF-α,IL-1βand IL-6 of SD rats in mild shock group,moderate shock group and severe shock group were significantly higher than those in control SD rats[HMGB1:(99.53±7.65),(165.26±9.56),(225.25±26.32)vs.(35.01±4.84)ng/ml;TNF-α:(44.32±3.01),
