艾灸联合情志护理干预对混合痔术后疼痛的影响

摘　　要：目的探讨艾灸及中药熏蒸的基础上给予情志护理干预对混合痔患者术后疼痛程度的影响。方法收集该院普通外科病房(2018年9月—2021年4月)行混合痔手术治疗的患者100例,按照入组先后顺序分为对照组(n=50)与观察组(n=50)。对照组给予艾灸肛周治疗+中药熏洗+常规护理干预,观察组在对照组的基础上给予情志护理干预。比较两组患者的疼痛程度[视觉模拟评分法(VAS)]、焦虑[汉密尔顿焦虑量表(HAMA)]与抑郁[汉密尔顿抑郁量表(HAMD)]程度,并对护理满意度(健康教育、护理质量、护理态度、护患关系、护理操作)及其术后相关并发症(肛门疼痛、便秘、出血、尿潴留)的发生率进行比较,最后比较两组患者疼痛消失时间及其创面愈合时间。结果观察组患者的疼痛程度(术后12 h、24 h及首次排便时)评分均低于对照组(P<0.05);两组患者的HAMA与HAMD评分均低于治疗前(P<0.05),且观察组低于对照组(P<0.05);观察组护理满意度均高于对照组(P<0.05);观察组术后相关并发症发生率低于对照组(P<0.05);观察组的疼痛消失时间及其创面愈合时间均短于对照组(P<0.05)。结论在艾灸及中药熏蒸的基础上给予情志护理干预能很好地减轻混合痔患者术后的疼痛程度,缓解其焦虑抑郁情绪,提升护理满意度,减少术后相关并发症的发生率。Objective To explore the effect of emotional nursing intervention on postoperative pain of patients with mixed hemorrhoids on the basis of moxibustion and fumigation with Chinese herbs.Methods One hundred patients with mixed hemorrhoids underwent surgical treatment between april and september of 2018 in the general surgical ward of our hospital were divided into control group(n=50)and observation group(n=50)according to the order of admission.The control group was given perianal moxibustion+important fumigation+routine nursing intervention,while the observation group was given emotional nursing intervention on the basis of the control group.The pain degree(visual assessment scale,VAS),anxiety degree(Hamilton anxiety scale,HAMA),and depression(Hamilton depression scale,HAMD)of the two groups after intervention were compared.The incidence of nursing satisfaction(health education,nursing quality,nursing attitude,nursing relationship,nursing operation)and postoperative complications(anal pain,constipation,bleeding and urinary retention)were compared,finally,the pain disappeared time and the wound healing time were compared between the two groups.Results The pain degree of the observation group was lower than that of the control group(12 h,24 h,after the first defecation)(P<0.05).HAMA and HAMD scores of the two groups were lower than before treatment(P<0.05),and the observation group was lower than the control group(P<0.05),and the nursing satisfaction of the intervention group was higher than the control group(P<0.05),the total complication rate in the intervention group was lower than that in the control group(P<0.05),and the time of pain disappearance and wound healing in the observation group was shorter than that in the control group(P<0.05).Conclusion Emotional nursing intervention on the basis of fumigation with acupuncture and traditional Chinese medicine can improve pain,relieve anxiety and depression and reduce complications in patients with mixed hemorrhoids.
