恩格列净联合二甲双胍治疗对2型糖尿病合并非酒精性脂肪肝患者visfatin水平的影响

摘　　要：目的观察恩格列净联合二甲双胍治疗初发2型糖尿病合并NAFLD对代谢相关指标及血清visfatin水平的影响。方法采用随机数字表法将研究对象分为二甲双胍治疗的对照组40例和二甲双胍联合恩格列净治疗的观察组40例,连续治疗12周,对比治疗前后WHR、BMI、FBG、PBG、HbA1C、血脂四项、HOMA-IR、Visfatin指标的差异。结果两组治疗后FBG、PBG、HbA1C、TC、TG、LDL-C、HOMA-IR、Visfatin均较治疗前显著降低,观察组下降幅度优于对照组,差异有统计学意义。观察组治疗后WHR、BMI、HDL-C较治疗前显著降低,差异有统计学意义。对照组WHR、BMI较治疗前有下降趋势,HDL-C有升高趋势,但无统计学意义。结论恩格列净联合二甲双胍治疗2型糖尿病合并NAFLD,可以有效控制血糖、改善胰岛素抵抗、纠正脂代谢紊乱和有效减重,可成为2型糖尿病合并NAFLD治疗新途径。Objective To observe the effect of engliazin combined with metformin on metabolism related indexes and serum visfatin level in patients with incipient type 2 diabetes mellitus with NAFLD.Methods The subjects were divided into the control group(n=40)treated with metformin and the observation group(n=40)treated with metformin combined with Engliazin.Treatment duration was 12 weeks.The differences of WHR,BMI,FBG,PBG,HbA1 C,blood lipid,HOMA-IR and Visfatin indexes were compared before and after treatment.Results After treatment,FBG,PBG,HbA1 C,TC,TG,LDL-C,HOMA-IR and Visfatin in the two groups were significantly lower than those before treatment,and the decrease extent in the observation group was more than that in the control group,the difference was statistically significant.The WHR,BMI and HDL-C of the observation group were significantly lower after treatment than before,and the differences were statistically significant.Compared with the pre-treatment period,there was a decreasing trend of WHR and BMI and an increasing trend of HDL-C in the control group,but there was no statistical significance.Conclusion Engliazin combined with metformin can effectively control blood glucose,improve insulin resistance,correct lipid metabolism disorder and effectively lose weight in the treatment of type 2 diabetes combined with NAFLD,which may be a new approach for the treatment of NAFLD in type 2 diabetes mellitus.
