糖尿病血瘀证与下肢动脉粥样硬化分级、临床指标的关系

摘　　要：目的观察糖尿病血瘀证与下肢动脉粥样硬化分级、临床指标的关系,为中医临床血瘀证辨证分型提供理论依据。方法选取糖尿病血瘀证患者140例、糖尿病非血瘀证患者164例,采集受试者双下肢动脉内-中膜厚度、斑块形成及管腔狭窄程度的彩超结果,计算血瘀证证候积分及下肢动脉粥样硬化积分,比较两组间年龄、病程、空腹血糖(FPG)、糖化血红蛋白(HbA_(1c))、血红蛋白(Hb)、收缩压(SBP)、舒张压(DBP)、血尿酸(BUA)、估算肾小球滤过率(eGFR)、胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL-C)、高密度脂蛋白(HDL-C),分析血瘀证积分与下肢动脉粥样硬化、临床指标分级的关系。结果糖尿病血瘀组血瘀积分与LDL-C呈正相关,与Hb呈负相关;与下肢动脉硬化严重程度呈正相关。结论糖尿病血瘀证与LDL-C、Hb、下肢动脉粥样硬化严重程度关系密切。Objective To observe the relationship between blood stasis score of diabetes mellitus and lower extremity athero-sclerosis classification and clinical indicators to provide a theoretical basis for TCM clinical blood stasis syndrome differentiation.Methods A total of 140 diabetes mellitus patients of blood stasis syndrome and 164 non-blood stasis diabetes mellitus patients.The color Doppler ultrasound results of the lower extremity arterial-media thickness,plaque formation and lumen stenosis were collected to calculate the blood stasis syndrome.The syndrome score and lower extremity atherosclerosis score,age,disease course,fasting blood glucose(FPG),glycosylated hemoglobin(HbA1c),hemoglobin(Hb),systolic blood pressure(SBP),dias-tolic blood pressure(DBP),blood uric acid(BUA),estimation of glomerular filtration rate(eGFR),cholesterol(TC),triglycerides(TG),low density lipoprotein(LDL-C)and high density lipoprotein(HDL-C)were compared.The relationship between scores of blood stasis syndrome and clinical indicators and lower extremity atherosclerosis scores was analyzed.Results The score of the diabetes mellitus patients of blood stasis syndrome was positively correlated with low-density lipoprotein and negatively correlated with hemoglobin.It was positively correlated with the severity of lower limb arteriosclerosis.Conclusion The diabetes mellitus of blood stasis syndrome is closely related to low-density lipoprotein,hemoglobin,and the severity of lower extremity atherosclerosis.
