脑络通胶囊联合阿加曲班治疗老年气虚血瘀证急性脑梗死的效果及对高凝和氧化应激的影响  

The Effect of Naoluotong Capsule Combined with Argatroban in the Treatment of Elderly Patients with Acute Cerebral Infarction and its Effect on Qi Deficiency and Blood Stasis Patients with Hypercoagulable State and Oxidative Stress

摘　　要：探讨脑络通胶囊联合阿加曲班治疗老年气虚血瘀证急性脑梗死(Acute cerebral infarction, ACI)的效果及对患者高凝状态和氧化应激的影响。选取2016年11月至2019年12月我院收治的104例老年气虚血瘀证ACI患者,按随机数字表法分为观察组与对照组,各52例。对照组静脉滴注阿加曲班注射液治疗,观察组在对照组基础上给予脑络通胶囊口服治疗,比较两组气虚血瘀证症状评分、神经功能缺损程度、临床疗效、凝血功能和氧化应激指标。治疗14 d后,观察组气虚血瘀证症状评分(半身不遂、口舌歪斜,言语蹇涩或不语,偏身麻木)及美国国立卫生研究院卒中量表(National institutes of health stroke scale, NIHSS)评分均低于对照组(P<0.05);观察组治疗总有效率为94.23%,高于对照组的78.85%(P<0.05);观察组纤维蛋白原(Fibrinogen, FIB)、平均血小板体积(Mean platelet volume, MPV)低于对照组,凝血酶原时间(Prothrombin time, PT)长于对照组(P<0.05);观察组髓过氧化物酶(Myeloperoxidase, MPO)低于对照组,谷胱甘肽过氧化物酶(Glutathione peroxidase, GSH-Px)、超氧化物歧化酶(Superoxide dismutase, SOD)高于对照组(P<0.05)。两组患者治疗期间未发生明显的不良反应。脑络通胶囊联合阿加曲班治疗老年气虚血瘀证ACI安全有效,可减轻患者临床症状和神经功能缺损程度,纠正机体血液高凝与氧化应激状态,提高预后效果。To investigate the effect of Naoluotong capsule combined with Argatroban in the treatment of elderly Qi deficiency and blood stasis patients with acute cerebral infarction(ACI) and the effect on hypercoagulability and oxidative stress, a total of 104 elderly Qi deficiency and blood stasis ACI patients admitted to the authors′ hospital from November 2016 to December 2019 were selected and divided into the observation group and the control group, 52 cases per group, according to the random number table method.The control group was treated with intravenous infusion of Argatroban, and the observation group was treated with Naoluotong capsule orally on the basis of the control group.The symptom score of Qi deficiency and blood stasis syndrome, the degree of neurological deficit, clinical efficacy, coagulation function and oxidative stress indexes were compared between 2 groups.After 14 days treatment, the symptom scores of Qi deficiency and blood stasis syndrome(hemiplegia, crooked tongue, slurred speech or silence, partial numbness) and National Institutes of Health stroke scale(NIHSS) scores of the observation group were lower than those of the control group(P<0.05).The total effective rate of treatment in the observation group was 94.23%,which was higher than 78.85% in the control group(P<0.05).The fibrinogen(FIB) and mean platelet volume(MPV) of the observation group were lower than those of the control group, and the prothrombin time(PT) was longer than the control group(P<0.05).The myeloperoxidase(MPO) of the observation group was lower than that of the control group, and glutathione peroxidase(GSH-Px) and superoxide dismutase(SOD) were higher than those of the control group(P<0.05).There had no serious side effects within 2 groups during treatment.Naoluotong capsule combined with Argatroban can effectively and safely treat elderly Qi deficiency and blood stasis ACI,which can reduce the clinical symptoms and neurological deficits of patients, correct the state of hypercoagulability and oxidative stress, and im
