自拟中药组方防治咽喉并发症的临床效果研究

摘　　要：目的探究自拟中药组方雾化吸入和针灸治疗对气管插管术后咽喉并发症的防治效果。方法选择2018年5月至2020年5月行全麻气管插管手术的腮腺肿瘤患者150例。按照随机分组法分三组:布地奈德组、自拟中药组及自拟中药+针灸组,每组50人。比较三组患者的咳嗽发生例数、声音嘶哑评分和咽痛伯格曼舒适度评分。结果半数患者咳嗽消失时间,自拟中药+针灸组<自拟中药组<布地奈德组(P<0.05)。声音嘶哑评分,术后D3,自拟中药+针灸组显著低于布地奈德组和自拟中药组(P<0.05);而术后D5,自拟中药+针灸组和自拟中药组显著低于布地奈德组(P<0.01)。咽痛评分,术后D3、D5和D7,布地奈德组均较自拟中药+针灸组及自拟中药组低(P<0.05),即咽痛情况较严重。结论自拟中药组方雾化吸入和针灸治疗可有效改善气管插管术后患者的咳嗽、声音嘶哑及咽痛症状,缩短患者咽喉并发症康复时间。Objective To explore the clinical effect of atomization inhalation of self-made traditional Chinese medicine and acupuncture in the prevention and treatment of pharynx and larynx complications in patients after endotracheal intubation.Methods The total of 150 patients with parotid tumor who underwent endotracheal intubation under general anesthesia from May 2018 to May 2020 were selected.According to the simple random grouping method,the patients were divided into three groups:budesonide group,self-made traditional Chinese medicine group,self-made traditional Chinese medicine+acupuncture treatment group,each group 50 cases.The number of cough cases,the score of hoarseness and the Bergman comfort scale(BCS) of pharyngeal pain were compared among the three groups.Results The time of cough disappearance in half patients in the self-made traditional Chinese medicine+acupuncture group was shorter than that in self-made traditional Chinese medicine group,which is shorter than budesonide group(P<0.05).At D3,the hoarseness score in the self-made traditional Chinese medicine+acupuncture group was significantly lower than that in the budesonide group and the self-made traditional Chinese medicine group(P<0.05);while D5 after operation,the hoarseness score of self-made traditional Chinese medicine+acupuncture group and the self-made traditional Chinese medicine group were significantly lower than that of the budesonide group(P<0.01).At D3,D5 and D7 after operation,the BCS scores in budesonide group were lower than those in self-made traditional Chinese medicine+acupuncture group and self-made traditional Chinese medicine group(P<0.05),the pharyngeal pain was serious in budesonide group.Conclusion The atomization inhalation and acupuncture treatment of self-made traditional Chinese medicine can effectively improve the symptoms of cough,hoarseness and pharyngeal pain sore of patients after endotracheal intubation,and shorten the rehabilitation time of pharynx and larynx complications.
