蜈倍汤中药溻渍联合氯地松乳膏治疗慢性湿疹的临床疗效

摘　　要：目的观察蜈倍汤中药溻渍联合氯地松乳膏治疗慢性湿疹血虚风燥证患者的临床疗效。方法将符合纳入标准的108例慢性湿疹血虚风燥证患者作为研究对象,随机分为中药组(基质乳膏外涂联合蜈倍汤溻渍外洗)、西药组(蜈倍汤安慰剂联合氯地松乳膏外涂)、中西药结合组(蜈倍汤溻渍联合氯地松乳膏外涂),每组36例,一日1次,2周为1个疗程,总疗程8周。于用药前和用药后的第2、4、6、8周采用特应性皮炎严重程度指数(scoring atopic dermatitis index,SCORAD)进行评分;观察并记录每次疗程的症状积分及治疗前后皮肤病生活质量指数(dermatology life quality index,DLQI)评分;观察中药及中西药结合治疗慢性湿疹的临床疗效及安全性。结果治疗8周后,中西药结合组总有效率(97.22%)优于中药组(86.11%)和西药组(83.33%),差异有统计学意义(P<0.05),而中药组与西药组疗效相当,无统计学差异(P>0.05);每个疗程,中西药结合组的SCORAD及DLQI评分均较中药组、西药组改善,差异有统计学意义(P<0.05)。结论蜈倍汤中药溻渍联合氯地松乳膏治疗慢性湿疹血虚风燥证,总体疗效及起效速度均优于单药治疗,且治疗过程中不良反应少,安全性高。Objective To observe the clinical effect of Wubei decoction TaZi therapy combined with dexamethasone ointment on chronic eczema patients with blood deficiency and wind dryness types.Methods The 108 patients with chronic eczema syndrome of blood deficiency and wind dryness types who met the inclusion criteria permit as the research object,were randomly divided into traditional Chinese medicine group(treated with matrix cream combined with Wubei decoction),western medicine group (treated with clodixone cream combined with placebo),Chinese and western medicine group(treated with Wubei decoction and clodixone cream),with 36 patients in each group,once a day,2 weeks for a course of treatment,the total course of treatment 8 weeks.Scoring atopic dermatitis index(SCORAD) was used to score before and at the secnd,fourth,sixth and eighth week after treatment.The symptom score and dermatology life quality index(DLQI) score of each course of treatment were observed and recorded.To observe the clinical efficacy and safety of traditional Chinese medicine and integrated traditional Chinese and western medicine in treating chronic eczema.Results After 8 weeks of treatment,the total effective rate of Chinese and western medicine group(97.22%) was better than that of traditional Chinese medicine group(86.11%) and western medicine group(83.33%),the difference was statistically significant(P<0.05).However,the curative effect of traditional Chinese medicine group was the same as that of western medicine group(P>0.05).In each course of treatment,the SCORAD and DLQI scores of Chinese and western medicine group were better than those of traditional Chinese medicine group and western medicine group,and the differences were statistically significant(P<0.05).Conclusion Wubei decoction TaZi therapy combined with dexamethasone ointment in the treatment of chronic eczema with blood deficiency and wind dryness types has better overall efficacy and onset speed than single drug treatment,and has less adverse reactions and safety in the treatment
