基于数据挖掘研究张明雪教授治疗冠心病合并高血压用药规律

摘　　要：目的探究张明雪教授治疗冠心病合并高血压的用药规律。方法收集张明雪教授于辽宁中医药大学附属医院在2019年1月1日—2021年3月31日诊治的冠心病合并高血压患者门诊处方,进行整理和筛选,通过古今医案云平台软V 2.3.5,采用频数分析、关联规则分析及聚类分析等数据挖掘方法,从而得出张明雪教授临床治疗冠心病合并高血压的用药规律。结果研究共纳入处方213首,涉及中药128味,其中使用频次超过80次的中药共18味,药性以平、寒、温为主;味多属甘、苦、辛;药物主归肺、肝、心经;复杂网络分析得出核心药物为黄芪、西洋参、法半夏、瓜蒌、薤白、黄芩、茯苓、柴胡、天麻、桔梗、龙骨、升麻、栀子、钩藤、夏枯草、赤芍、甘草、决明子。结论张明雪教授治疗冠心病合并高血压药物性味多以平、寒、温、甘、苦、辛为主,多归肺、肝、心经,以补虚泻实、调整气血阴阳为治则,以益气宣痹、平肝潜阳、活血化瘀、健脾化痰为治法。Objective To study the medication rule of Professor ZHANG Mingxue in the treatment of coronary heart disease with hypertension.Methods The outpatient prescriptions of patients with coronary heart disease and hypertension diagnosed and treated by Professor ZHANG Mingxue in the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from January 1,2019 to March 31,2021,were sorted and screened.Through the ancient and modern medical records cloud platform soft v 23.5 and the data mining methods such as frequency analysis,association rule analysis and cluster analysis,we can get the medication law of Professor ZHANG Mingxue in the clinical treatment of coronary heart disease complicated with hypertension.Results A total of 213 prescriptions were included in this study,involving 128 Chinese medicines,of which 18 Chinese medicines were used more than 80 times.The properties of the medicines are mainly gentle,cold and warm;the tastes are mostly sweet,bitter and pungent.The medicines mainly belong to the lung,liver and heart meridians.The complex network analysis shows that the core drugs were Huangqi(Radix Astragali),Xiyangshen(Radix Panacis Quinquefolii),Fabanxia(Rhizoma Pinelliae Praeparatum),Gualou(Fructus Trichosanthis),Xiebai(Bulbus Allii Macrostemonis),Huangqin(Radix Scutellariae),Fuling(Poria),Chaihu(Radix Bupleuri),Tianma(Rhizoma Gastrodiae),Jiegeng(Radix Platycodonis),Longgu(Fossil Fragments),Shengma(Rhizoma Cimicifugae),Zhizi(Fructus Gardeniae),Gouteng(Uncariae Ramulus cum Uncis),Xiakucao(Spica Prunellae),Chishao(Radix Paeoniae Rubra),Gancao(Radix et Rhizoma Glycyrrhizae)and Juemingzi(Semen Cassiae).Conclusion Professor ZHANG Mingxue′s treatment of coronary heart disease complicated with hypertension is mostly gentle,cold,warm,sweet,bitter and pungent drugs which mostly belong to lung,liver and heart meridians.The treatment principle is tonifying deficiency and reducing excess,adjusting Qi,blood Yin and Yang,and the treatment method is supplementing Qi and dispersing Bi syndrome,calming li
