摘　　要：目的观察病灶清除、敏感抗生素联合牡丹叶方熏洗治疗下肢慢性骨髓炎的临床效果。方法选取山东省日照市中医医院2015年1月至2020年6月收治的下肢慢性骨髓炎患者80例,采用随机数字表法将其分为对照组和观察组,各40例。对照组采用常规的手术病灶清除及敏感抗生素抗感染治疗,观察组在对照组的基础上行牡丹叶方熏洗治疗。治疗6周后,比较两组临床疗效。比较两组治疗前后视觉模拟评分法(VAS)评分及白细胞计数、C反应蛋白、红细胞沉降率变化。记录两组不良反应发生情况。结果两组临床总有效率比较,差异无统计学意义(P>0.05)。治疗后,两组VAS评分、白细胞计数、红细胞沉降率、C反应蛋白均低于治疗前,且观察组VAS评分、红细胞沉降率、C反应蛋白低于对照组,差异有统计学意义(P<0.05)。两组均未出现严重的不良反应。结论病灶清除、敏感抗生素联合牡丹叶方熏洗的中西医结合疗法能有效缓解患者疼痛,促进下肢慢性骨髓炎局部病灶的愈合,是一种安全而有效的措施。Objective To observe the clinical efficacy cavernostomy and sensitive antibiotics combined with fumigation and washing with Mudanye Decoction in the treatment of chronic osteomyelitis of lower limbs.Methods Eighty patients with chronic osteomyelitis of lower limbs admitted to Rizhao Hospital of Traditional Chinese Medicine,Shandong Province from January 2015 to June 2020 were selected and divided into control group and observation group by random number table method,with 40 cases in each group.The control group was treated with conventional surgical cavernostomy and sensitive antibiotic anti-infection treatment,while the observation group was treated with fumigation and washing with Mudanye Decoction on the basis of control group.After six weeks of treatment,the clinical efficacy of the two groups was compared.Visual analog scale(VAS)score,white blood cell count,C-reactive protein,and erythrocyte sedimentation rate were compared between the two groups before and after treatment.The incidence of adverse reactions in the two groups was recorded.Results There was no significant difference in total clinical effective rate between the two groups(P>0.05).After treatment,VAS score,white blood cell count,erythrocyte sedimentation rate,and C-reactive protein in two groups were lower than those before treatment,and VAS score,erythrocyte sedimentation rate,and C-reactive protein of observation group were lower than those of control group,the differences were statistically significant(P<0.05).There were no serious adverse reactions in both groups.Conclusion The integration of traditional and Western medicine therapy of cavernostomy and sensitive antibiotics combined with fumigation and washing with Mudanye Decoction can effectively relieve pain and promote healing of local lesions of chronic osteomyelitis of lower limbs,which is a safe and effective measure.
