从太阴脾、肺探讨糖尿病高危足与足痿的关系

摘　　要：本文先从脾与胰关系溯源,得出脾、胰在中医范畴内统一关系的论点;再根据脾、胰主消化吸收的现代生理特点,推演出其二者在中医藏象学共同的太阴脾土属性,进一步从“消渴”“脾瘅”病机方面阐述了太阴脾土与糖尿病的密切关系。随后通过梳理“瘀”与太阴肺的内在联系,探讨了糖尿病足周围神经病变和周围血管病变与“瘀”的病理机制,分析了糖尿病高危足以及中医足痿的临床特点,指出中医“足痿”与现代医学糖尿病高危足的高度相关一致性,最后立足于“治痿独取阳明”以及“阳明太阴相表里”的经论,剖析了“足痿”与藏象太阴脾、肺之间的病因病机。本文初步从藏象学太阴视角对糖尿病高危足进行探讨,为临床从太阴防治糖尿病足提供中医理论支持。First of all,the origin of the relationship between spleen and pancreas is traced,and the unified relationship between spleen and pancreas in the scope of traditional Chinese medicine is concluded in this paper.According to the modern physiological characteristics of digestion and absorption of spleen and pancreas,this paper deduced the common properties of spleen and spleen in TCM then,and further elaborated the close relationship between spleen and diabetes from the TCM area of“quench thirst”and“Dan spleen”pathogenesis.Subsequently,by combing the internal relationship between“stasis”and the lung of taiyin,to investigate the pathological mechanism of diabetic foot peripheral neuropathy,peripheral vascular disease and“stasis”.Finally,the clinical characteristics of diabetic high-risk foot and Chinese medicine“foot impotence”were analyzed,and the high correlation between Chinese medicine impotence and modern medicine diabetic high-risk foot was pointed out.Based on the theory of“treating impotence by yangming alone”and“Yangming Taiyin as the outside and inside of each other”,the etiology and pathogenesis of“foot impotence”and spleen and lung of Taiyin zang manifestation theory were analyzed,and to provide TCM theoretical support for clinical prevention and treatment of diabetic foot from the perspective of taiyin,we preliminarily discussed the high-risk diabetic foot from the perspective of taiyin zang manifestation theory.
