摘 要:目的探讨脓毒症患者血清微小RNA-23b(miR-23b)、微小RNA-146a(miR-146a)、吲哚胺-2,3-双加氧酶(IDO)水平变化及其临床价值。方法选择2018年1月至2019年1月该院确诊的105例重症监护病房脓毒症患者作为脓毒症组,另选择103例同期体检健康者作为对照组。检测所有研究对象血清miR-23b、miR-146a、IDO水平,比较脓毒症组和对照组、不同病情严重程度和预后脓毒症患者的血清miR-23b、miR-146a、IDO水平,分析血清miR-23b、miR-146a、IDO水平间的相关性及这3项指标对脓毒症的诊断价值。结果与对照组相比,脓毒症组血清miR-146a、IDO水平升高,miR-23b水平降低,差异均有统计学意义(P<0.05)。不同病情严重程度脓毒症患者血清miR-23b、miR-146a、IDO水平两两比较,差异均有统计学意义(P<0.05)。脓毒症死亡患者血清miR-146a、IDO水平高于脓毒症存活患者,而miR-23b水平低于脓毒症存活患者,差异均有统计学意义(P<0.05)。Pearson相关分析结果显示,血清miR-23b水平与miR-146a、IDO均呈线性负相关(r=-0.618、-0.623,P<0.001),miR-146a水平与IDO呈线性正相关(r=0.552,P<0.001)。受试者工作特征曲线分析结果显示,血清miR-23b、miR-146a、IDO联合检测诊断脓毒症的曲线下面积为0.846,高于miR-23b、miR-146a、IDO单项检测诊断脓毒症的0.796、0.808、0.734,差异均有统计学意义(P<0.05)。结论脓毒症患者血清miR-146a、IDO水平升高,miR-23b水平降低,这3项指标密切相关,可能参与了脓毒症的病情进展,可为临床诊断、治疗脓毒症提供依据。Objective To investigate the changes and clinical value of the serum levels of microRNA-23b(miR-23b),microRNA-146a(miR-146a)and indoleamine 2,3-dioxygenase(IDO)in patients with sepsis.Methods A total of 105 patients with sepsis diagnosed in this hospital from January 2018 to January 2019 in the intensive care unit were selected as the sepsis group,and 103 healthy individuals who underwent physical examination during the same period were selected as the control group.The serum levels of miR-23b,miR-146a and IDO were detected in all subjects,and the serum levels of miR-23b,miR-146a and IDO of the sepsis group and the control group,the patients with different disease severity and prognosis were compared.The correlation among serum miR-23b,miR-146a and IDO levels and the diagnostic value of the three indicators for sepsis were analyzed.Results Compared with in the control group,the serum levels of miR-146a and IDO in the sepsis group were increased,and the level of miR-23b was decreased,and the differences were statistically significant(P<0.05).There were statistically significant differences in the serum levels of miR-23b,miR-146a and IDO in patients with different severity of sepsis(P<0.05).The serum levels of miR-146a and IDO in patients who died of sepsis were higher than those in patients who survived sepsis,while the level of miR-23b in patients who died of sepsis was lower than that in patients who survived sepsis,and the differences were statistically significant(P<0.05).Pearson correlation analysis results showed that serum level of miR-23b was linearly negatively correlated with miR-146a and IDO(r=-0.618,-0.623,P<0.001),and level of miR-146a was linearly positively correlated with IDO(r=0.552,P<0.001).The receiver operating characteristic curve analysis results showed that the area under the curve of the combined detection of serum miR-23b,miR-146a and IDO in the diagnosis of sepsis was 0.846,which was higher than that of the single detection of miR-23b,miR-146a and IDO in the diagnosis of sepsis,and the di
