摘　　要：目的分析半乳糖凝集素-3(Gal-3)、环氧合酶-2(COX-2)在异位内膜组织中的表达水平,其与子宫内膜异位症(EM)患者腹腔镜手术治疗预后的关系,以及其对EM患者手术治疗预后不良风险的预测价值。方法选取新疆克拉玛依市中心医院2019年1月至2020年5月择期行腹腔镜手术治疗的107例EM患者为研究对象,患者均行异位内膜组织Gal-3、COX-2表达水平检测,且术后随访1年。以术后随访1年内复发情况作为预后观察指标,将复发患者作为预后不良组,无复发患者作为预后良好组。采集患者相关基线资料,分析异位内膜组织中Gal-3、COX-2表达水平与EM患者腹腔镜手术治疗预后的关系及其对患者手术治疗预后不良风险的预测价值。结果随访1年内,107例EM患者中预后不良42例,占39.25%。预后不良组r-AFS分期Ⅲ~Ⅳ期患者占比及Gal-3、COX-2表达水平均高于预后良好组(P<0.05)。Spearman双变量相关分析结果显示,EM患者异位内膜组织中Gal-3表达水平与COX-2呈正相关(r=0.426,P<0.001)。Logistic回归模型分析结果显示,r-AFS分期为Ⅲ~Ⅳ期及Gal-3、COX-2表达水平升高可能是EM患者腹腔镜手术治疗预后不良的独立危险因素(OR>1,P<0.05)。受试者工作特征曲线分析结果显示,异位内膜组织中Gal-3、COX-2单独及联合检测预测EM患者腹腔镜手术治疗预后不良风险的曲线下面积大于0.800,均有一定预测价值。结论EM患者腹腔镜手术治疗预后不良可能与异位内膜组织中Gal-3、COX-2表达水平升高有关,且Gal-3、COX-2预测EM患者腹腔镜手术治疗预后不良风险有一定价值。Objective To analyze the expression levels of galectin-3(Gal-3)and cyclooxygenase-2(COX-2)in ectopic endometrial tissue,their relationship with the prognosis of patients with endometriosis(EM)after laparoscopic surgery,and their predictive value for the risk of poor prognosis in EM patients undergoing surgical treatment.Methods A total of 107 patients with EM who underwent elective laparoscopic surgery in Karamay Central Hospital of Xinjiang from January 2019 to May 2020 were selected as the research objects,and the patients were detected for Gal-3 and COX-2 in ectopic endometrial tissues and received one-year postoperative follow-up.The recurrence within one-year postoperative follow-up was used as a prognostic indicator,and the patients with recurrence were regarded as the poor prognosis group,and the patients without recurrence were regarded as the good prognosis group.The baseline data of patients were collected,and the relationship between the expression levels of Gal-3 and COX-2 in ectopic endometrial tissue and the prognosis of EM patients undergoing laparoscopic surgery and their predictive value for the risk of poor prognosis in patients with surgical treatment were analyzed.Results Within one year of follow-up,among the 107 EM patients,42 cases had poor prognosis,accounting for 39.25%.The proportion of patients with r-AFS stageⅢ-Ⅳand the expression levels of Gal-3 and COX-2 in the poor prognosis group were higher than those in the good prognosis group(P<0.05).Spearman bivariate correlation analysis results showed that the expression level of Gal-3 in ectopic endometrial tissue of EM patients was positively correlated with COX-2(r=0.426,P<0.001).Logistic regression model analysis results showed that r-AFS stageⅢ-Ⅳand elevated expression levels of Gal-3 and COX-2 may be independent risk factors for poor prognosis in EM patients treated with laparoscopic surgery(OR>1,P<0.05).Receiver operating characteristic curve analysis results showed that the area under the curve of Gal-3 and COX-2 in ectopic e
