摘  要:目的探讨血清抗β_(2)-糖蛋白1抗体(aβ_(2)GP1)在自身免疫性卵巢早衰患者中的表达及其与性激素水平的相关性。方法选取该院2017年5月至2020年1月收治的114例卵巢早衰患者及62例同期体检健康女性作为研究对象,将其中的62例自身免疫性卵巢早衰患者作为试验组,52例非自身免疫性卵巢早衰患者作为对照组,62例同期体检健康女性作为健康组。检测并比较3组血清aβ_(2)GP1表达情况和性激素[血清雌二醇(E2)、卵泡刺激素(FSH)、黄体生成素(LH)]水平,采用Pearson相关分析血清aβ_(2)GP1阳性率与性激素水平的相关性,采用Logistic回归模型分析发生自身免疫性卵巢早衰的独立危险因素。结果试验组aβ_(2)GP1-免疫球蛋白(Ig)M和aβ_(2)GP1-IgG的阳性率均明显高于对照组和健康组(P<0.05)。与对照组和健康组比较,试验组E2、LH水平明显降低(P<0.05),FSH水平明显升高(P<0.05)。aβ_(2)GP1-IgM和aβ_(2)GP1-IgG的阳性率与患者E2、LH水平呈负相关(P<0.05),与FSH水平呈正相关(P<0.05)。Logistic回归模型分析结果显示,血清aβ_(2)GP1阳性,E2、LH水平降低及FSH水平升高是发生自身免疫性卵巢早衰的独立危险因素(OR>1,P<0.05)。结论血清aβ_(2)GP1在自身免疫性卵巢早衰患者中呈高表达,其阳性率与性激素水平有关,是发生自身免疫性卵巢早衰的独立危险因素。Objective To investigate the expression of serum anti-β_(2)-glycoprotein 1 antibody(aβ_(2)GP1)in patients with autoimmune premature ovarian failure,and its correlation with sex hormone level.Methods A total of 114 patients with premature ovarian failure who were admitted to this hospital from May 2017 to January 2020 and 62 healthy women who underwent physical examination during the same period were selected as the research objects.Among them,62 patients with autoimmune premature ovarian failure were selected as the experimental group,52 patients with non-autoimmune premature ovarian failure were selected as the control group,and 62 healthy women who underwent physical examination during the same period were selected as the healthy group.The expression of serum aβ_(2)GP1 and the levels of sex hormones[serum estradiol(E_(2)),follicle-stimulating hormone(FSH),and luteinizing hormone(LH)]were detected and compared among the three groups.Pearson correlation was used to analyze the correlation between serum aβ_(2)GP1 positive rate and sex hormone levels,and Logistic regression model was used to analyze the independent risk factors of autoimmune premature ovarian failure.Results The positive rates of aβ_(2)GP1-immunoglobulin(Ig)M and aβ_(2)GP1-IgG in the experimental group were significantly higher than those in the control group and the healthy group(P<0.05).Compared with in the control group and the healthy group,the levels of E2 and LH in the experimental group significantly decreased(P<0.05),and the level of FSH significantly increased(P<0.05).The positive rates of aβ_(2)GP1-IgM and aβ_(2)GP1-IgG were negatively correlated with the levels of E2 and LH(P<0.05),and they were positively correlated with the level of FSH(P<0.05).Logistic regression model analysis showed that serum aβ_(2)GP1 positive,decreased E2 and LH levels and increased FSH level were independent risk factors for autoimmune premature ovarian failure(OR>1,P<0.05).Conclusion Serum anti-aβ_(2)GP1 is highly expressed in patients with autoimmune
