基于“血不利则为水”理论探讨宫颈癌术后下肢淋巴水肿辨治思路

摘　　要：下肢淋巴水肿的根本病机是正虚为本,痰、湿、瘀互结。在具体治疗上,以“血不利则为水”为切入点,以扶正祛邪、血水同调为原则,使活血而不伤正,养血而不留瘀。且不可拘泥于单纯的活血利水法,要审证求因,急则治其标、缓则治其本,并根据患者临床表现的不同,辨证施治,将活血利水法同益气养血、养阴清热、温阳化湿、化痰通络、疏肝健脾等法巧妙地结合应用,调整脏腑阴阳,达到机体阴平阳秘的状态。The fundamental pathogenesis of lower extremity lymphedema is righteousness Qi deficiency-based combination of Phlegm,Dampness and blood stasis.In terms of specific treatment,the starting point should be based on the theory that"unsmooth blood circulation resulting in Water",and the principle that strengthening the righteousness Qi and eliminating pathogenic factors as well as achieving the harmony between blood and Water should be taken,so as to activate the blood without harming the righteousness Qi,and nourish the blood without letting the stasis be.Also,we should not stick to the simple method of promoting blood circulation and invigorating Water.We should examine the symptom,the syndrome and seek the causes,and then treat the symptoms in an emergency or treat the root cause if the symptoms is not rather urgent.According to the different clinical manifestations of patients,the treatment should be taken based on syndrome differentiation.The methods of nourishing Yin and clearing Heat,warming Yang and removing Dampness,removing Phlegm and dredging collaterals,soothing the Liver and strengthening the Spleen are skillfully combined and applied to adjust Yin and Yang of the Zang-Fu viscera,so as to achieve the balance between Yin and Yang of the body.
