健脾化湿方对慢性分泌性中耳炎患者鼓膜置管术后咽鼓管功能的影响

摘　　要：目的:观察健脾化湿方对脾虚湿蕴型慢性分泌性中耳炎(chronic secretory otitis media,CSOM)鼓膜置管术后咽鼓管功能的影响。方法:70例CSOM患者按照随机数字表法随机分为观察组和对照组各35例。两组患者均给予鼓膜置管术治疗,观察组在此基础上给予健脾化湿方。比较两组患者临床疗效及置管术前、置管术后第1个月、3个月、6个月、9个月时的咽鼓管功能障碍问卷-7评分、咽鼓管功能评分。结果:与术前比较,观察组术后第1个月、3个月、6个月、9个月的咽鼓管功能障碍问卷-7评分均显著降低(P<0.01);组间比较:观察组术后第1个月、3个月、6个月、9个月咽鼓管功能障碍问卷-7评分低于同期对照组(P<0.05)。治疗后,两组患者术后第1个月、3个月、6个月、9个月咽鼓管功能评分均显著升高(P<0.05);观察组术后第1个月、3个月、6个月、9个月咽鼓管功能评分均显著高于同期对照组(P<0.05)。两组临床疗效分布经秩和检验,差异有统计学意义(Z=-2.479,P=0.013)。结论:健脾化湿方可显著改善患者咽鼓管功能,提高患者生活质量。Objective:To observe the effect of Jianpi Huashi Recipe on Eustachian tube function after tympanic tube placement in chronic secretory otitis media(CSOM)with Spleen deficiency and Dampness accumulation.Methods:70 CSOM patients were randomly divided into observation group and control group with 35 cases in each group.Both groups were treated with tympanic membrane catheterization,and the observation group was treated with Jianpi Huashi Recipe on this basis.The clinical efficacy and the Eustachian tube dysfunction questionnaire-7 score and Eustachian tube function score before and 1,3,6,and 9 months after catheter placement were compared between the two groups.Results:Compared with preoperative,the Eustachian Tube Dysfunction Questionnaire-7 scores of observation group were significantly decreased at 1,3,6,and 9 months after operation(P<0.01);comparison between groups:observation group after operation 1,3 The scores of Eustachian Tube Dysfunction Questionnaire-7 at 6 and 9 months were lower than those of the control group at the same period(P<0.05).After treatment,the Eustachian tube function scores of the two groups at 1,3,6,and 9 months after operation were significantly increased(P<0.05);The scores of tympanic tube function were significantly higher than those of the control group at the same period(P<0.05).The distribution of clinical efficacy between the two groups was tested by rank sum test,and the difference was statistically significant(Z=-2.479,P=0.013).Conclusion:Jianpi Huashi Recipe can significantly improve the function of the Eustachian tube and improve the quality of life of the patients.
