摘 要:目的 探究急性心肌梗死(AMI)患者血清长链非编码RNA肺腺癌转移相关转录本1(lncRNA MALAT1)表达与心肌损伤、凝血功能的相关性。方法 将2020年1月—2021年1月广元市中心医院确诊的313例冠心病患者分为冠心病组(n=163)和AMI组(n=150)。收集患者一般资料;采用实时荧光定量PCR法测定血清lncRNA MALAT1水平;采用酶联免疫吸附法(ELISA)测定血清心肌损伤指标肌酸激酶同工酶(CK-MB)、心型脂肪酸结合蛋白(H-FABP)水平;采用全自动血凝仪检测凝血功能指标活化部分凝血酶时间(APTT)、血浆凝血酶原时间(PT)、血浆凝血酶时间(TT)水平;采用Pearson法分析血清lncRNA MALAT1水平与心肌损伤、凝血功能指标相关性;采用多因素Logistic回归分析影响冠心病患者发生AMI的影响因素。结果 与冠心病组患者比较,AMI组患者实验室指标Hs-CRP、高密度脂蛋白及血清lncRNA MALAT1水平均升高(P<0.05),其余资料比较差异无统计学意义(P>0.05);血清lncRNA MALAT1水平高表达是冠心病患者发生AMI的独立危险因素(P<0.05);与冠心病组比较,AMI组心肌损伤指标CK-MB、H-FABP水平及凝血功能指标APTT、PT、TT水平均降低(P<0.05);患者血清lncRNA MALAT1水平与心肌损伤指标CK-MB、H-FABP水平及凝血功能指标APTT、PT、TT水平均呈负相关(P<0.05)。结论 lncRNA MALAT1在急性心肌梗死患者血清中异常高表达,可能与患者心肌损伤及凝血功能异常有关。Objective To explore the relationship between the expression of serum long noncoding RNA lung adenocarcinoma metastasis-associated transcript 1(lncRNA MALAT1) in patients with acute myocardial infarction(AMI) and myocardial injury and coagulation function.Methods A total of 313 patients with coronary heart disease diagnosed in Guangyuan Central Hospital from January 2020 to January 2021 were divided into coronary heart disease group(n=163) and AMI group(n=150).General patient data were collected.Serum lncRNA MALAT1 levels were determined by real-time quantitative PCR.Enzyme-linked immunosorbent assay(ELISA) was used to determine serum myocardial injury markers creatine kinase isoenzyme(CK-MB) and heart fatty acid binding protein(H-FABP) level.Automatic blood coagulation instrument was used to detect the levels of activated partial thrombin time(APTT),plasma prothrombin time(PT) and plasma thrombin time(TT),which were indicators of coagulation function.The Pearson method was used to analyze the correlation between serum lncRNA MALAT1 level and myocardial injury and coagulation function indexes.Multivariate Logistic regression analysis was used to analyze the influencing factors of AMI in patients with coronary heart disease.Results Compared with patients with coronary heart disease,the levels of hs-CRP,high-density lipoprotein and serum lncRNA MALAT1 of the AMI group were all significantly higher than those of the coronary heart disease group(all P<0.05).The levels of CK-MB,H-FABP and APTT,PT,and TT of the AMI group were all significantly lower than those of the coronary disease group(all P<0.05).Logistic trgtrssion analysis showed that high level of serum lncRNA MALAT1 was an independent risk factor for AMI in patients with coronary heart disease(P<0.05),and the level of lncRNA MALAT1 was negatively correlated with CK-MB,H-FABP,APTT,PT and TT(all P<0.05).Conclusion The abnormal expression of lncRNA MALAT1 in the serum of AMI patients may be related to myocardial injury and abnormal coagulation function.
