摘 要:目的:分析孤立性小脑后下动脉夹层(PICAD)高分辨磁共振管壁成像的特点。方法:回顾性收集8例经临床诊断为孤立性PICAD的患者,分析其临床和磁共振影像资料。结果:8例PICAD患者中男7例,女1例,年龄31~65岁;均为单侧发病,左侧1例,右侧7例;1例急性期,4例亚急性早期,3例亚急性晚期。PICAD高分辨磁共振管壁成像特点如下:①血管外径扩张伴壁间血肿;②血管外径串珠样改变并腔内血栓形成;③管腔增粗并腔内血栓形成;④壁间血肿或腔内血栓多呈明显强化。结论:PICAD在高分辨磁共振管壁成像上具有一定特征性。高分辨磁共振管壁成像有助于PICAD的诊断,值得在临床工作中推广使用。Aim:To investigate the features of isolated posterior inferior cerebellar artery dissection(PICAD)on high-resolution magnetic resonance vessel wall imaging(HRMR-VWI).Methods:Eight patients with clinically diagnosed isolated PICAD were included in this study,and their clinical and magnetic resonance imaging data were analyzed.Results:There were 8 patients with PICAD,including 7 males and 1 female,aged 31-65 years.All the cases were unilateral,including 1 case on the left side and 7 cases on the right side.One case was in acute stage,4 in early subacute stage,and 3 in late subacute stage.The characteristics of PICAD on HRMR-VWI were summarized as follows,vessel dilatation with intramural hematoma,"pearl and string sign"with intraluminal thrombosis;lumen dilatation and intraluminal thrombosis;intramural hematoma and intraluminal thrombus were often significantly enhanced.Conclusion:There are some certain characteristics of PICAD on HRMR-VWI,which is helpful in the diagnosis of PICAD.HRMR-VWI is worthy of promotion and application in clinical work.
