正骨十四法联合中药外敷在桡骨远端骨折中的应用效果分析

摘　　要：目的研究探讨正骨十四法联合舒筋定痛散外敷治疗桡骨远端骨折的临床疗效。方法选取2019年1月至2021年1月郑州市骨科医院收治的100例桡骨远端骨折患者作为研究对象,按照随机数表法将其随机分为观察组(50例)与对照组(50例)。观察组患者采用正骨十四法联合舒筋定痛散外敷治疗,对照组患者单纯采用正骨十四法治疗,对比观察两组患者功能训练开始时间、肿胀消退时间、骨折愈合时间、腕关节活动度以及腕关节功能。结果观察组患者功能训练开始时间、肿胀消退时间、骨折愈合时间均明显短于对照组(t=6.809、4.761、4.859,P均<0.001);治疗后3个月,观察组患者腕关节旋前、旋后、掌屈活动度均明显大于对照组(t=20.384、22.656、20.190,P均<0.001);治疗后6个月,观察组患者腕关节功能为优者7例、良者23例、可者17例、差者3例,与对照组患者腕关节功能为优者4例、良者19例、可者18例、差者9例无明显差异(Z=-1.800,P=0.072)。结论正骨十四法联合舒筋定痛散外敷治疗桡骨远端骨折,可有效促进肿胀消退,缩短骨折愈合时间,改善腕关节活动度,值得临床推广应用。Objective To study the clinical efficacy of bone-setting manipulation with fourteen techniques combined with external application of Shujin Dingtong Powder in the treatment of distal radius fractures.Methods 100 patients with distal radius fracture,admitted to Zhengzhou Orthopaedic Hospital from January 2019 to January 2021,were enrolled as research subjects and divided,using the random number table,into observation group(n=50)and control group(n=50).Patients in observation group were treated with the bone-setting manipulation with fourteen techniques combined with external application of Shujin Dingtong Powder,whereas patients in control group were treated with the bone-setting manipulation with fourteen techniques alone.The time of starting functional exercise,time for swelling resolution,fracture healing time,wrist range of motion and wrist functions were compared between the two groups.Results The time of starting functional exercise,time for swelling resolution and fracture healing time of patients in observation group were all significantly shorter than control group(t=6.809,4.761 and 4.859,all P<0.001).After 3 months of treatment,the ranges of motion at pronation and supination of wrist joint and at palmar flexion of patients in observation group were obviously greater than control group(t=20.384,22.656 and 20.190,all P<0.001).After 6 months of treatment,the wrist joint functions were evaluated as excellent in 7 cases,good in 23 cases,fair in 17 cases and poor in 3 cases in observation group,which showed no significant difference compared with control group(Z=-1.800,P=0.072)-excellent in 4 cases,good in 19 cases,fair in 18 cases and poor in 9 cases.Conclusion The combined application of bone-setting manipulation with fourteen techniques and external application of Shujin Dingtong Powder in the treatment of distal radius fracture can effectively facilitate the resolution of swelling,shorten healing time of fracture,and improve the range of motion of wrist joint.
