清瘀方督脉熏蒸联合清热化瘀方保留灌肠治疗慢性盆腔炎的临床研究

摘　　要：目的观察清瘀方督脉熏蒸联合清热化瘀方保留灌肠治疗慢性盆腔炎(CPID)的临床疗效。方法选择2020年3月至2021年3月北京市通州区中西医结合医院妇产科门诊CPID患者90例,按照随机数字表法分为2组,各45例。对照组给予常规西药治疗,观察组在对照组基础上给予清瘀方督脉熏蒸联合清热化瘀方保留灌肠治疗。观察2组疗效及症状体征恢复情况,比较2组治疗前后中医证候评分、炎症指标、血液流变学指标变化情况。结果观察组总有效率95.56%(43/45),对照组总有效率82.22%(37/45),治疗组疗效优于对照组(P<0.05)。观察组性交疼痛消失时间、白带正常时间、盆腔病灶消失时间均短于对照组(P<0.05)。观察组治疗后中医证候各项评分均低于对照组(P<0.05)。观察组治疗后白细胞计数(WBC)、中性粒细胞比例及C反应蛋白(CRP)、降钙素原(PCT)水平均低于对照组(P<0.05)。观察组治疗后全血黏度(高切)、全血黏度(低切)、血浆黏度、红细胞沉降率(ESR)均低于对照组(P<0.05)。结论清瘀方督脉熏蒸联合清热化瘀方保留灌肠治疗CPID疗效显著,能够有效改善症状体征,缓解炎症状态。Objective To observe the clinical effect of fumigation with Qingyu Formula at Du Mai combined with retention enema with Qingre Huayu Formula for chronic pelvic inflammatory disease(CPID).Methods From March 2020 to March 2021,totally 90 CPID patients in the Department of Obstetrics and Gynecology Clinic of our hospital were randomly divided into the observation group(n=45)and the control group(n=45);in addition to conventional Western medicine,and the observation group received fumigation with Qingyu Formula at Du Mai combined with retention enema with Qingre Huayu Formula.The aim was to compare curative effect,recovery of symptoms and signs,traditional Chinese medicine(TCM)syndrome score,inflammatory index,hemorheology index.Results The overall effective rate was 95.56%(43/45)in the observation group,higher than that of 82.22%(37/45)in the control group(P<0.05).After treatment,the disappearing time of sexual intercourse pain,recovery time of abnormal leucorrhea,disappearance time of pelvic lesions were decreased in the observation group compared with the control group(P<0.05);TCM syndrome scores in the observation group were decreased in comparison with the control group(P<0.05).Compared with the control group,white blood cell count(WBC),percentage of neutrophil,C-reactive protein(CRP),procalcitonin(PCT)in the observation group were decreased(P<0.05);whole blood high shear viscosity,whole blood low shear viscosity,plasma viscosity,erythrocyte sedimentation rate(ESR)in the observation group were decreased(P<0.05).Conclusion For CPID patients,fumigation with Qingyu Formula at Du Mai combined with retention enema with Qingre Huayu Formula has significant curative effect,can effectively improve symptoms and signs,and relieve inflammatory state.
