中药药油走罐法联合NB-UVB照射治疗银屑病临床观察

摘　　要：[目的]观察中药药油走罐法联合NB-UVB照射治疗寻常型斑块状银屑病的疗效及对血清肿瘤坏死因子α(TNF-α)、白细胞介素17(IL-17)、白细胞介素23(IL-23)水平的影响。[方法]将符合纳入标准的60例患者,随机分为治疗组30例、对照组30例。治疗组予中药药油走罐联合NB-UVB照射治疗,对照组单用NBUVB照射治疗。治疗8周后,比较两组银屑病面积和严重程度指数(PASI)及血清TNF-α、IL-17、IL-23的水平变化,并评估疗效。[结果]治疗后两组的PASI评分及血清TNF-α、IL-17、IL-23水平均明显下降,但治疗组下降更显著(P<0.05)。治疗组总有效率为86.67%,对照组为66.67%,两组疗效比较,差异有统计学意义(P<0.05)。[结论]中药药油走罐联合NB-UVB照射治疗银屑病疗效明显优于单用NB-UVB照射,其机制可能是通过降低血清TNF-α、IL-17、IL-23等炎症因子水平来达到治疗目的。[Objective]To observe the curative effect of moving cupping method with medicated oil combined with NB-UVB irradiation in the treatment of psoriasis,to observe the levels of serum TNF-α,IL-17,IL-23 and explore its mechanism.[Methods]Sixty patients met the inclusion criteria were randomly divided into the treatment group and the control group,with 30 cases for each.The treatment group was treated with moving cupping combined with NB-UVB irradiation,while the control group was treated with NB-UVB irradiation alone.After 8 weeks of treatment,the PASI scores and the levels of serum TNF-α,IL-17 and IL-23 were compared between the two groups,and the curative effect was evaluated.[Results]After treatment,the PASI score and serum TNF-α,IL-17 and IL-23 levels in the two groups were significantly decreased,but the decrease was more significant in the treatment group(P<0.05).The total effective rate was 86.67%in the treatment group and 66.67%in the control group.The difference was statistically significant(P<0.05).[Conclusion]The efficacy of moving cupping of medicated oil combined with NB-UVB irradiation in the treatment of psoriasis is significantly better than that of NB-UVB irradiation alone.The mechanism may be achieved by reducing the levels of serum TNF-α,IL-17,IL-23 and other inflammatory factors.
