摘 要:目的探讨与分析小柴胡汤联合抗菌药物对耐碳青霉烯类鲍曼不动杆菌(CRAB)致腹膜炎/脓毒症大鼠模型的影响。方法36只CRAB致腹膜炎/脓毒症大鼠随机分为3组,模型组、西药组与联合用药组,每组12只。西药组给予皮下注射头孢哌酮/舒巴坦[240/120 mg/(kg·12 h)]+亚胺培南[156 mg/(kg·12 h)],2次/天;联合用药组在西药组治疗的基础上给予小柴胡汤中药提取液0.3 mg/kg灌胃,2次/天;模型组每日灌胃生理盐水。结果西药组与联合用药组治疗第3天与第7天的肺髓过氧化物酶(MPO)水平及干湿指数均低于模型组(P<0.05),联合用药组低于西药组(P<0.05)。西药组与联合用药组治疗第3天与第7天的血清肿瘤坏死因子-α(TNF-α)和白细胞介素-6(IL-6)水平均低于模型组(P<0.05),联合用药组低于西药组(P<0.05)。西药组与联合用药组治疗第3天与第7天的肺Notch1、端粒酶调节相关蛋白(TRAP)蛋白相对表达水平均低于模型组(P<0.05),联合用药组低于西药组(P<0.05)。结论小柴胡汤联合抗菌药物在CRAB致腹膜炎/脓毒症大鼠模型的应用能抑制Notch1、TRAP蛋白的表达,抑制炎症因子的表达,从而降低肺MPO水平与干湿指数。Objective To investigate and analyze the effect of Xiaochaihu decoction combined with antibacterial agents on carbapenem-resistant Acinetobacter baumannii(CRAB)-induced peritonitis/sepsis rat model.Methods A total of 36 CRAB-induced peritonitis/septic rats were equally randomly divided into model group,western medicine group and combined medication group,12 rats in each group.The western medicine group were given subcutaneous injection of cefoperazone/sulbactam[240/120 mg/(kg·12 h)]+imipenem[156 mg/(kg·12 h)]twice daily;the combined medication group were given Xiaochaihu decoction Chinese medicine extract 0.3 mg/kg by gavage on the basis of the western medicine group,twice daily;the model group were gavaged with normal saline every day.Results The level of lung myeloperoxidase(MPO)and dryness and wetness index on the 3rd and 7th day of treatment in the western medicine group and combined medication group were lower than those in the model group(P<0.05),and those in the combined medication group were lower than those in the western medicine group(P<0.05).Serum tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)levels on the 3rd and 7th day of treatment in the western medicine group and combined medication group were lower than those in the model group(P<0.05),and those in the combined medication group were lower than those in the western medicine group(P<0.05).The relative expression levels of Notch1 and telomerase regulation associated protein(TRAP)protein in the lungs in the western medicine group and the combined medication group were lower than those in the model group on the 3rd and 7th day of treatment(P<0.05),and those in the combined medication group were lower than those in the western medicine group(P<0.05).Conclusion The application of Xiaochaihu decoction combined with antibacterial agents in CRAB-induced peritonitis/sepsis rat model could inhibit the expression of Notch1 and TRAP protein,inhibit the expression of inflammatory factors,and thereby reduce lung MPO level and dryness and wetness inde
