摘  要:目的探究高压氧联合脑苷肌肽治疗创伤性颅脑损伤患者的效果及其对缺氧性诱导因子-1α(HIF-1α)及微小RNA(miR)-210表达的影响。方法选取山西省晋城市人民医院2018年4月至2020年4月收治的创伤性颅脑损伤患者110例,根据随机数字表法将其分为对照组与观察组,各55例。两组均给予常规治疗,对照组给予脑苷肌肽治疗,观察组给予高压氧联合脑苷肌肽治疗。两组均治疗30 d。比较两组治疗前后格拉斯哥昏迷量表(GCS)评分、血清神经元特异性烯醇化酶(NSE)、可溶性凋亡相关因子(sFas)、HIF-1α及miR-210水平,随访1、3个月继发性癫痫发生情况。结果本研究最终纳入102例患者,对照组52例,观察组50例。治疗30 d后,两组GCS评分均较治疗前升高,且观察组高于对照组(P<0.05)。治疗30 d后,两组血清NSE、sFas、HIF-1α及miR-210水平均较治疗前降低,且观察组低于对照组(P<0.05)。随访3个月,观察组继发性癫痫发生率低于对照组(P<0.05)。结论高压氧联合脑苷肌肽治疗创伤性颅脑损伤的临床效果更好,能明显减轻神经功能损伤,有较强抑制神经细胞凋亡的作用,调节HIF-1α及miR-210表达量,在一定程度上可降低短期继发性癫痫的发生风险。Objective To explore the effect of hyperbaric oxygen combined with cattle encephalon glycoside and ignotin in treating patients with traumatic brain injury and its effect on the expression of hypoxia inducible factor-1α(HIF-1α)and microRNA(miR-210).Methods A total of 110 patients with traumatic brain injury treated in Jincheng People’s Hospital of Shanxi Province from April 2018 to April 2020 were selected and divided into control group and observation group according to random number table method,with 55 cases in each group.Both groups were given conventional treatment,control group was treated with cattle encephalon glycoside and ignotin,and observation group was treated with hyperbaric oxygen combined with cattle encephalon glycoside and ignotin.Both groups were treated for 30 days.Glasgow coma scale(GCS)score,serum levels of neuron specific enolase(NSE),soluble factors associated suicide(sFas),HIF-1α,and miR-210 were compared between two groups before and after treatment.The incidence of secondary epilepsy was followed up for one and three months.Results A total of 102 patients were enrolled in this study,including 52 in the control group and 50 in the observation group.Thirty days after treatment,GCS scores of two groups were higher than those before treatment,and observation group was higher than control group(P<0.05).Thirty days after treatment,serum levels of NSE,sFas,HIF-1α,and miR-210 of two groups were lower than those before treatment(P<0.05),and observation group was lower than control group(P<0.05).Three months after treatment,the incidence of secondary epilepsy in observation group was lower than that in control group(P<0.05).Conclusion Hyperbaric oxygen combined with cattle encephalon glycoside and ignotin has a better clinical effect in the treatment of traumatic brain injury.It can significantly reduce nerve function injury,inhibit nerve cell apoptosis and regulate the expression of HIF-1α and miR-210,and can reduce the risk of short-term secondary epilepsy to a certain extent.
