摘 要:目的通过测量相关数据评估各分型尿道上裂患儿的盆底结构异常,初步探讨盆底结构异常的意义。方法回顾性分析本院2014年11月至2019年10月所收治的16例尿道上裂患者及8例对照组资料,阴茎体型尿道上裂组5例,年龄12~35个月,平均20个月;阴茎耻骨型尿道上裂组11例,年龄14~40个月,平均19个月。对照组8例(睾丸肿瘤),年龄4~35个月,平均13个月。比较三组在磁共振检查横断面、冠状面及矢状面中盆底结构数据上是否存在差异。结果阴茎体型尿道上裂组在耻骨直肠角、膀胱颈-骶骨夹角及肛管-骶骨夹角与对照组差异显著(P<0.05)。阴茎耻骨型尿道上裂组在耻骨间距、膀胱颈-骶骨夹角与对照组差异显著(P<0.05)。两组尿道上裂组与对照组在年龄上经卡方检验比较,结果分别为χ^(2)=0.063,P=0.654无统计学差异(P>0.05)。结论通过MRI测量儿童盆底参数对各分型尿道上裂患儿的盆底结构进行多方位对比。明确各分型尿道上裂儿童的盆底发育异常。有助于临床术前诊断、制定手术方案及预后的判断。Objective To evaluate the abnormal pelvic floor structure of children with various types of epispadias by measuring relevant data,and to explore the significance of abnormal pelvic floor structure.Methods A retrospective analysis was conducted on 16 patients with epispadias and 8 cases of control group treated in our hospital from November 2014 to October 2019,including 5 cases in the penile type epispadias group,with an average age of about 20 months(December to 35 months);There were 11 cases of penile pubic epispadias,with an average age of about 19 months(14-40 months).There were 8 cases(testicular tumors)in the control group,with an average age of 13 months(april-35 months).We compared whether there were differences in the data of pelvic floor structure in cross-sectional,coronal and sagittal planes of the three groups.Results All 24 children were male.There were significant differences in puborectal angle,bladder neck sacral angle and anal canal sacral angle between the penile type epispadias group and the control group(P<0.05).There were significant differences in pubic distance and bladder neck sacral angle between penile pubic epispadias group and control group(P<0.05).Chi square test showed that there was no significant difference in age between the two groups(χ^(2)=0.063,P=0.654,P>0.05).Conclusion The pelvic floor parameters of children were measured by MRI,and the pelvic floor structures of children with various types of epispadias were compared in multiple directions.To determine the abnormal development of pelvic floor in children with various types of epispadias.It is helpful for clinical preoperative diagnosis,making operation plan and judging prognosis.
