摘　　要：目的对比泼尼松与β-七叶皂苷钠对急性面神经炎患者功能恢复及肌电图的影响。方法选择2017年6月至2019年4月于复旦大学附属中山医院青浦分院神经外科住院部接受治疗的94例急性面神经炎患者,随机分为对照组(常规治疗+泼尼松)和观察组(常规治疗+β-七叶皂苷钠),每组47例,常规治疗疗程为28 d,其他治疗从14 d后开始,总疗程28 d。对比两组临床疗效、治疗前后面神经功能评分、肌电图、House-Brackman(H-B)分级及安全性。结果比较对照组与观察组总有效率(93.62%vs 97.87%),差异无统计学意义(P>0.05)。两组面神经功能评分均较治疗前上升,但组间比较差异无统计学意义(P>0.05)。两组面神经复合肌动作电位波幅均上升,R1波潜伏期均下降,组间比较差异无统计学意义(P>0.05)。两组H-B分级情况差异无统计学意义(P>0.05)。观察组不良反应总发生率低于对照组(8.51%vs 27.66%),差异有统计学意义(P<0.05)。结论泼尼松与β-七叶皂苷钠治疗急性面神经炎的效果无显著差异,均可有效改善患者面神经功能及肌电图,β-七叶皂苷钠安全性相对较佳。Objective To compare the effects of prednisone andβ-aescinate on functional recovery and electromyography in patients with acute facial neuritis.Methods A total of 94 patients with acute facial neuritis treated in the inpatient department of neurosurgery,Qingpu Branch of Zhongshan Hospital Affiliated to Fudan University from June 2017 to April 2019 were randomly divided into control group(routine treatment+prednisone)and observation group(routine treatment+β-aescinate),47 cases in each group.The routine treatment course was 28 days,and other treatments began after 14 days,with a total course of 28 days.The clinical efficacy,neurological function score before and after treatment,electromyography,House-Brackman(H-B)grade and safety of the two groups were compared.Results There was no significant difference in the total effective rate between the control group and the observation group(93.62%vs 97.87%,P>0.05).The facial nerve function scores in both groups were higher than those before treatment,but there was no significant difference between the two groups(P>0.05).The amplitude of action potential of facial nerve complex muscle increased and R1 wave latency decreased in both groups,but there was no significant difference between the two groups(P>0.05).There was no significant difference in H-B grading between the two groups(P>0.05).The total incidence of adverse reactions in the observation group was lower than that in the control group(8.51%vs 27.66%),and the difference was statistically significant(P<0.05).Conclusion There was no significant difference betweenβ-aescinoside and prednisone in the treatment of acute facial neuritis,both of which could effectively improve facial nerve function and electromyography,andβ-aescinoside was relatively safe.
